Insurance Claim Procedure. By signing this Agreement, I acknowledge that I have read and understand the following insurance claim procedure and understand that my failure to adhere to the requirements of the procedure may result in claims being denied by Xavier University or sent to a collection agency. Xavier University requires that each student be covered by a Health Insurance Plan (the “Primary Plan”. Health insurance provides students with access to the health care system and to necessary medical services in order to support their academic success. Students are automatically enrolled in the student health insurance plan and a health insurance charge is placed on the Bursar bill. If I seek a waiver from the student health insurance plan, it is my responsibility to determine if my private health insurance plan will be accepted by health care providers at Xavier (i.e. TriHealth) and those off-campus which may be necessary for my well-being. If my private insurance does not provide such coverage, I shall be responsible for all medical bills. The NCAA does not permit Xavier University to either provide coverage or pay for expenses related to illness/conditions which are not sustained as the direct result of any accident in our sports program. Any charges related to treatment of a medical illness carried out by an outside vendor that are not covered by the Primary Plan will become the responsibility of the student. All insurance claims must be processed through my primary insurance policy before Xavier University can file a claim and that the student-athlete will have 104 weeks from the date of the injury to have any surgical procedures or necessary services performed. I understand that ▇▇▇▇▇▇ will not necessarily be responsible for any/all deductibles. How to File a Claim: (1) Submit the bills incurred to your family/employer group coverage plan first. If your claim is denied and you receive a letter of denial, proceed to step three (3). (2) If there remains a balance after your insurance company has contributed to the claim, send the EXPLANATION OF BENEFITS/END OF BALANCE STATEMENT from the insurance company and a copy of the itemized bills incurred to the Xavier University Sports Medicine Department. (3) If you receive a letter of denial from your family/employer group insurance or plan administrator, then send the letter of denial and a copy of the bills incurred to the Xavier University Sports Medicine Department.
