REQUESTS/APPROVAL In order to be granted leave, Association bargaining unit members shall submit a “Leave Notification of Request/Approval Form” which will cover all Temporary and Extended Leaves and the conditions for notification, request and approval. The “Leave Notification of Request/Approval Form” shall be accessible from the District website.
