An LOA will be required before VERIZON will process an order for Services provided in cases in which the Customer currently receives local exchange or Exchange Access service from VERIZON or from a CLEC other than SPRINT. Such LOA may be a blanket LOA or such other form as agreed upon between VERIZON and SPRINT.

GTE will not release the Customer service record (CSR) or inquiry containing Customer proprietary network information (CPNI) to USA Digital on GTE Customer accounts unless USA Digital first provides to GTE a written LOA. Such LOA may be a blanket LOA or other form agreed upon between GTE and USA Digital authorizing the release of such information to USA Digital or if state or federal law provides otherwise, in accordance with such law. An LOA will be required before GTE will process an order for Services provided in cases in which the Customer currently receives local exchange or Exchange Access service from GTE or from a CLEC other than USA Digital. Such LOA may be a blanket LOA or such other form as agreed upon between GTE and USA Digital.
LETTER OF AUTHORIZATION (LOA. Each Party is responsible for obtaining an LOA from each End User that requests LNP from one Party to the other Party. Both Parties agree to adhere to the applicable federal and/or state requirements regarding LOAs and preferred carrier freezes.

Notwithstanding any other provision of this Agreement, the Parties’ negotiations over the terms of a mutually agreeable blanket letter of authorization shall not delay, impede, or interrupt the provision of number portability by either Party.

Such LOA may be a blanket LOA or such other form as agreed upon between GTE and Gulf Coast.
LETTER OF AUTHORIZATION (LOA a Health First Letter of Authorization to release the insured party's information to a newly nominated agent for Group lines of business. The Agent of Record may receive copies of quoting information, renewal rates, and monthly billing information.
