Miscellaneous Policies. Pets, excluding service animals, are not permitted inside the facility or back patio. • CONB PARD is not responsible for items left behind, before, during or after a rental. • CONB PARD staff may enter the facility at any time on any occasion. • CONB PARD reserves the right to take photographs of rentals for its own records and for use in the future. • ▇▇▇▇▇▇ must make sure that the number of guests does not exceed the occupancy max for the facility. • Music played inside the Facility should be kept at a volume to be heard only by the guests inside the Haus. By ordinance, music is required to cease at 10:00 pm.

Clean-up is the Renter’s responsibility. Renter will provide sufficient supervision of guests to minimize food and beverages spills during the event. CMP will supply appropriate cleaning materials and Renter is responsible for the following: • Decorations: Remove all decorations and personal belongings. • Keys: Lock doors and return keys to proper location. • Tables: Cleaned off and wiped down. • Chairs: Place in original orderly or stack, and wiped down if necessary. • Floors: Vacuum rugs, floor and restroom area, where necessary (vacuum provided) • Trash: Remove trash from all receptacles (including restrooms). Replace with provided garbage bags. Dispose of trash in dumpsters near Cy’s east entrance. (Hwy 41 side of the building). • Lights: Turn off lights. (Light by restrooms remains “ON” at all times.) • Keys: Lock up and return key(s) to proper location. Initial/Date If Renter provides alcoholic beverages, proof of insurance will be required. • Renter shall provide proof of homeowner’s insurance policy that carries Host Liquor Liability coverage (this is usually a standard within homeowner policies) OR shall take out, at their expense, general liability insurance naming Chaska Mill Properties as an additional insured. • A copy of the insurance coverage must be submitted to CMP at least 30 days prior to the event. CMP holds the right to restrict Renter from providing alcoholic beverages or to cancel the event and retain 50% of the payment if proof of insurance is not provided 30 days prior to the event.

Miscellaneous Policies.  Supervisions/Consultations: PTS does on-going case supervision/consultations/trainings with other professionals to ensure safety and thoroughness in the therapy process. PTS maintains the right to consult their attorney at any time and for any issue. Client is responsible for reimburse for these fees.  Court/Legal Matters: If legal involvement is required, PTS requires payment of an advance, non-refundable fee of $5,000.00. In addition to the advance non-refundable fee, you will be responsible for the time PTS expends on your behalf at the rate $150.00 per hour, and you will also be responsible for all other out-of-pocket expenses that PTS incurs on your behalf. PTS does not assess for ▇▇▇▇▇▇▇’▇ comp, disability, or any other related matter that involves extra paperwork.  Client records are kept in accordance with current statutes that govern mental health professionals.  EMDR:  If you are considering legal action regarding your incident or event and/or Traumatic Brain Injury or Eye issues, please notify the therapist so it can be discussed prior to the initiation of EMDR therapy.  While obtaining EMDR therapy from PTS, your full compliance with treatment recommendations made by the EMDR therapist, neurologist and/or psychiatrist, and full compliance with the recommendations of your attorney (if any), are mandatory. Failure to comply fully with such recommendations may result in your dismissal as a client.  When agreeing to EMDR therapy, you are committing that you will be prompt and that you will come regularly for scheduled sessions.  It may take several sessions before the therapist can determine if you are eligible for EMDR which may mean referral to another therapist is needed.  EMDR may recover memories that a client is unaware, which may contain uncomfortable feelings. RELEASE OF INFORMATION AND AUTHORIZATION TO PAY INSURANCE BENEFITS I authorize Practical Therapeutic Solutions, PLLC to apply for insurance benefits. I authorize payment directly to Practical Therapeutic Solutions, PLLC. I authorize any release of any necessary information to, and communication with, all other health care treatment providers for the purposes of processing my (my child’s) claim. I also authorize a copy of this authorization to be used in place of the original.

Miscellaneous Policies. 1) The undersigned “Renter” MUST be 21 years of age to enter into this agreement or this agreement is void and all funds exchanged will not be refunded. 2) Parking for attendees of stated event at Downtown Toledo Event Center is limited to the available parking lots. Parking can never be guaranteed. Any violations of the ▇▇▇▇▇ County parking laws are the responsibility of the undersigned “Renter”. 3) Downtown Toledo Event Center Management must approve decorations or other special requests that impede or attach to the facility. Any decorations left at the facility will be discarded. 4) All candles are required to have glass ▇▇▇▇▇▇▇ or hurricane lamps. No open flames allowed. 5) All decorations provided by the “Renter” are to be removed from the facility immediately following the event. 6) If Downtown Toledo Event Center is providing linens (including but not limited too: table clothes, chair covers, chair sashes, table runners, and overlays) then final order details need to be provided to Downtown Toledo Event Center 30 days prior to the event. 7) If “Renter” contracts to use backstage Audio/Visual Screen then they are subject to a fee of $250.
Miscellaneous Policies. The following are miscellaneous policies grouped together for efficiency:  At no time should the parent(s)/guardian(s) discuss any differences or conflicts that they might have with New Beginnings with their child. The student should never be used as a mediator in these situations. If a parent/guardian has any questions or concerns regarding any aspect of their child’s enrollment at New Beginnings, he/she should address them to the administration of New Beginnings.  At no time should the parent(s)/guardian(s) convey information to their child and then require your child to keep this information from New Beginnings.  New Beginnings will not administer any behavior modification medication. No student will be enrolled who is required to take such medication. No such medication will be left on campus for the student to take.  Only clear contacts are acceptable at New Beginnings. Anything else will be returned. If your child wears contact lenses, you will need to provide a back-up pair of eyeglasses in case something happens to the contacts.  New Beginnings, or any of its staff, are not responsible for any harm or injury occurring to your child during your child’s stay at New Beginnings.  The parent(s)/guardian(s) agree(s) to execute an Application for Admission to New Beginnings.
