Licenses and Fees. Upon the Practice Operator’s request, Manager shall cause the payment to be made for professional licensure fees and board certification fees of Physicians associated with the Practice Operator. Any such expenses shall be Practice Operating Expenses in accordance with Section 1.43 on Exhibit 1.

The Seller shall obtain all licenses and permits (other than the license and permit granted by the Agreement) required to conduct the activities pursuant to this Agreement and shall promptly pay all fees, taxes or other state or municipal costs necessary for said licenses, permits or activities.

B. The AIRPORT AUTHORITY is required under the Department of Transportation (DOT) Motor Carrier Title 49 DFR Part 382 to establish procedures to attain and maintain an Alcohol and Drug-Free Workplace.
1. The UNION has negotiated a policy with the AIRPORT AUTHORITY, meeting the terms of the above referenced DOT regulation, which will automatically incorporate any modifications or revisions as required during the term of the Agreement.

Licenses and Fees. License fees are due 30 days from the date of each invoice. The fees for any additional Licensed VMs shall be the same as provided herein unless CSC has given You 60 days’ prior written notice of a pricing change, at which time the pricing change will be effective as of the date in the notice and thereafter. The Licensed VM Quantity specifies the maximum number of concurrently running Virtual Machines (VMs) under the management of the Software, summed across all of Your Instances of the Software. Additional VM Licenses may be licensed from CSC or Reseller, as applicable, for the applicable Price per VM, under separate Order(s). If You are currently receiving Maintenance & Support Services under an Order, then the applicable Maintenance and Support fees will be applied to the additional VM Licenses, prorated to the end of the current Maintenance Term.

b. At the sole discretion of the City, an employee may be required to obtain any physical examination from a physician designated by the City.
2. The City shall designate a limited number of positions within the Non-Supervisory Unit which shall require a Commercial Driver's License. All employees required to hold a Commercial Driver's License have the responsibility to maintain the Commerical Driver's License and a Medical Card and the City shall pay fees as specified in Section 1 above.
a. Such positions will be eligible for a license premium pay of twenty dollars ($20.00) per pay period. Effective July 1, 2006, it is the intent of the parties to phase out license premium pay. Said license premium pay shall be paid to those employees who were required, as a condition of employment, to hold a valid Commercial Driver's License and who were receiving license premium pay prior to July 1, 2006. Employees hired or required to obtain a Commercial Driver's License after July 1, 2006, shall not be eligible for license premium pay.
b. Those receiving the license premium pay have the responsibility for keeping current the Commercial Driver's License and a Medical Card and the City shall pay fees as specified in Section 1 above.
c. Eligibility for such positions shall be based upon the following criteria:
(1) Priority will be given to employees currently holding a valid Commercial Driver's License.
(2) If there are more employees holding a license than positions available which require the license, assignment to such positions shall be based upon relative seniority within the class.
(3) If there are insufficient employees available for assignment who currently hold a Commercial Driver's License, then eligibility for obtaining a license shall be based upon relative seniority within the class.
