– Modified Restricted Duty. An employee who is on sick leave or Worker’s Compensation leave may request that he/she be placed in a temporary modified restricted duty assignment that the employee is qualified to perform. Such requests shall be evaluated on a case-by-case basis in order to determine whether the University can make the requested accommodations in light of the employee’s medical restrictions and/or availability of work. The University may demand proof from the University’s physician or the employee’s physician that the employee is physically able to perform the modified restricted duty assignment in question without significant risk that such an assignment will aggravate any pre-existing injury, and that there is a reasonable expectation that the employee will be able to assume full duties and responsibilities within ninety (90) days. If an employee is unable to assume his or her full duties and responsibilities within ninety (90) days, the University retains the right to return the employee to inactive status on either workers compensation or sick leave, unless an extension is approved based on an evaluation with University of Chicago Occupational Medicine (UCOM) and/or the employee’s physician. Such extensions will be made temporarily and periodically evaluated on a month-to-month basis. Nothing in this Section is intended to require the University to create a modified restricted duty assignment. The University will email the Union’s President when informed that an employee has been released to modified restrictive duty.

  • Permitted License Uses and Restrictions (a) This Order Form Supplement allows you, as an authorized User under the Master Subscription Agreement, to use the Software on any Supported Device and on no other devices.
