Notice Packet. The Notice Packet will provide that the Class Members who wish to object to the Settlement may do so in writing, signed, dated, and mailed to the Settlement Administrator postmarked no later than the Response Deadline.

Notice Packet. The Class Notice, Exclusion Form, and Objection Form. AA. Objection Form: The Notice of Objection Form, substantially like the form attached hereto as Exhibit C, subject to Court approval.
Notice Packet. The Notice Packet will provide that Class Members who wish to exclude themselves from the class action Settlement must mail to the Settlement Administrator an Exclusion Form. The written request for exclusion must: (a) include the Class Member’s name, address, and last four digits of the social security number; (b) be addressed to the Settlement Administrator; (c) be signed by the Class Member; and (d) be postmarked no later than the Response Deadline.

The Settlement Administrator will have the Class Notice translated to Spanish and other languages as necessary after the Court preliminarily approves the English version of the Class Notice. The Class Notice will include, non- exclusively, information regarding the nature of the Lawsuit; a summary of the substance of the Settlement’s terms; the Class definition; the procedure for objecting to the Settlement, the date for the Final Approval Hearing, and the formula used for the Settlement Shares. The Class Notice shall include the Class Member’s estimated Settlement Share.
