On Unpaid Leave. 1. Health, life, optical, dental. An employee may purchase group insurance upon request for a maximum of twelve (12) months at group rates. After that time there is an option for conversion. If an employee on leave terminates employment or the leave is not renewed, the employee may enroll for COBRA benefits at group rates plus a two (2%) percent administration fee for eighteen (18) months. The premiums must be paid to the school district on or before five (5) days prior to the first of the month for which the premium is being paid. For example, the premium must be paid on or before November 25 for December insurance benefits.
