Optical Insurance Benefit. The City shall provide for err.ployees and eligible members of employee's famil~' an Optical Plan as provided by the Co-op Optical Service in their plan dated October 14, 1976. COpies of the plan will be given to the Union. The plan in general provides every employee, spouse and all dependent children under the age of nineteen (19) an o[)tometric refraction and glasses, if needed, once every two (2) years. The plan details the type of frames and lenses available. Retired employees shall continue to be covered by this plan, cost sustained by the City, until the retired employee reaches age 65 or is eligible for Medi-Care. Should an employee, active or retired, become deceased, said err.xxxxxx's spouse and eligible dependents under the plan shall continue to be covered by said insurance, provided said spouse remains unmarried. Retired employees who obtain emIJloyn,ent from an employer who ~Jrovides Optical Insurance shall not be covered by the Cit~;'s Optical Insur- ance for duration of such employment.

Retirees, including spouse and eligible dependents, shall continue to be covered by this plan, cost sustained by the City, prior to Medicare eligibility. Retirees who obtain employment from an employer who provides optical insurance shall not be covered by the City’s optical insurance plan for the duration of such employment. Should an employee, either active or retired, become deceased, said employee’s spouse and eligible dependents under the plan shall continue to be covered by said insurance, provided said spouse remains unmarried. For employees hired after July 1, 2015, retiree optical insurance option will be eliminated.
