PARTICIPATION IN CREDENTIALING. 28 Provider shall participate in North Sound BH-ASO’s credentialing and re-credentialing process 29 that shall satisfy, throughout the term of this Agreement, all credentialing and re- 31 North Sound BH-ASO of any change in the information submitted or relied upon by Provider 32 to achieve credentialed status. If Provider’s credentialed status is revoked, suspended, or 33 limited by North Sound BH-ASO, North Sound BH-ASO may, at its discretion, terminate this 34 Agreement and/or reassign individuals to another provider.

PARTICIPATION IN CREDENTIALING. Provider shall participate in GRBH-ASO’s credentialing and re-credentialing process as outlined in GRBH-ASO policies and procedures that shall satisfy, throughout the term of this Contract, all credentialing and re-credentialing criteria established by MCOs. Provider shall immediately notify GRBH-ASO of any change in the information submitted or relied upon by Provider to achieve credentialed status. If Provider’s credentialed status is revoked, suspended or limited by GRBH-ASO.GRBH-ASO may, at its discretion, terminate this Contract and/or reassign clients to another provider.

Provider shall accept delegation of credentialing responsibilities for Provider’s subcontractors at Health Plan’s request and shall cooperate with Health Plan in establishing and maintaining appropriate credentialing mechanisms within Provider’s organization. If delegation of credentialing responsibilities to Provider is revoked, Health Plan shall reduce any otherwise applicable payments owing to Provider. Provider will successfully complete a facility site review by Health Plan, if deemed necessary by Health Plan in accordance with the Medi-Cal Agreement. Provider shall at all times comply with the credentialing requirements set forth in Attachment E. Provider Manual. Provider shall comply and render Covered Services in accordance with the contents, instructions and procedures set forth in Health Plan’s Provider Manual, which may be amended from time to time. Health Plan’s Provider Manual is incorporated in this Agreement by this reference.

  • Program Participation By participating in the CRF Program, Grantee agrees to: a. Not increase any Eligible Household’s rent through January 2021; b. Waive all costs, fees and charges incurred by Eligible Households as a result of non- payment or partial payment of rent during the impacted months; c. Not consider non-payment or partial payment by Eligible Households during impacted months when considering renewal of an Eligible Household’s lease, or, share this information with other rental properties, credit bureaus and tenant screening companies; d. Not initiate new Eligible Household evictions for non-payment of rent and must suspend all pending evictions of Eligible Households for nonpayment of rent for the duration of the rental payment assistance; e. Not issue a notice to vacate to Eligible Households for nonpayment of rent until the end of the Eviction Relief Period; and f. Not require Eligible Households to vacate the unit until 30 days after such notice.

  • Participation Contributor will participate in any Work Groups (defined in the Policy) identified above, and any other Work Groups that it actually participates in or notifies ODIF that it wants to participate in, according to the rules and procedures in the then-current OpenID Process Document (“Process Document”), which is fully incorporated into this Agreement by this reference, and subject to the Policy.
