Practitioner. Authorizing Practitioner hereby represents that neither Authorizing Practitioner, the company under which Authorizing Practitioner submits claims to Medicare (Group Practice) nor, to the best Authorizing Practitioner’s knowledge, Group Practice’s employees, agents or independent contractors involved in the provision of services have been excluded from participation in any Federally-funded health care programs, including, but not limited to, Medicare and Medicaid.
Practitioner. FTNTT Faculty members from academia, industry, and/or other professional fields whose primary role is to deliver instruction or service in professional programs and applied areas (e.g., music, journalism, architecture, librarianship, flight).
Practitioner. An insolvency practitioner for the purposes of the Insolvency Act 1986 (or, in relation to jurisdictions outside the United Kingdom, any similar official).
Practitioner. Any temporary, probationary or permanent member, part-time or full- time, covered by this collective bargaining agreement.
Practitioner. 14.2.1 Will not other than as may be necessary during the performance period, communicate the Performer's contact address and telephone number to any other party and shall at all times respect the privacy of the Performer and the confidentiality of any personal information or details in possession of the Practitioner;
14.2.2 Shall ensure that a fully completed agreement shall be in the possession of the Performer or Agent as indicated in S2 not less than forty eight (48) hours before the required performance unless under exceptional circumstances;
14.2.3 Warrants that it will endeavour to ensure that the Production House or any sub-contractor with whom the Practitioner may be in a contractual relationship shall comply with the provisions of this agreement.
14.2.4 It is agreed that each of the warranties constitutes material terms of the agreement and should any party at any stage wish to prove the contrary, the onus of such proof shall rest on such party
Practitioner. Practitioner is licensed to practice their services on the Premises in accordance with State law. Practitioner agrees that it is their responsibility to keep their license(s) current in accordance with the State where the Premises is located. If for any reason the Practitioner practices their Services without a valid license, the Practitioner agrees to indemnify the Employer and accept all legal and financial responsibility of such acts.
Practitioner. A licensed or otherwise appropriately qualified health care professional or entity who is qualified and, when applicable, duly licensed and/or certified by the state in which he, she or it is located to furnish Covered Services when acting within the scope of his, her or its license or certification.
Practitioner. A licensed allopathic or osteopathic physician or other nonphysician health practitioner who is a member of Practitioner Group and has agreed to provide medical care and health related services to Members in accordance with the terms and conditions of this Agreement.
Practitioner. Practitioner means the sponsor as to its Employer clients of the Volume Submitter Plan and as defined in Section 13.04 of Rev. Proc. 2005-16.
Practitioner. In relation to practitioner services, practitioner refers to physicians, advanced registered nurse practitioners, physician assistants, registered nurse first assistants, and anesthesiology assistants, where appropriate, according to the individual’s scope of practice.
