Production of Medical Certificates. A medical certificate is required for any absence where the duration is more than two (2) consecutive rostered shifts or where the absence occurs immediately before or after a public holiday. In addition, a medical certificate is required where absences total more than four (4) single days in a year, based on anniversary of employment or as required by NRMA Motoring and Services given the frequency and pattern of absences. Employees applying for personal leave shall have regard to the trust and responsibility given to them by NRMA Motoring and Services. Where this trust and responsibility is ignored or abused, NRMA Motoring and Services may suspend paying personal leave. Circumstances where this may be deemed necessary include: (i) a pattern of single day or short duration absences; or (ii) a pattern of absences immediately before or after a public holiday, weekend or period of leave, and (iii) For the purposes of (i) and (ii) above, when the leave is taken for reasons that are not due to illness, injury or emergency effecting the employee or a member of the employees immediate family or household. NRMA Motoring and Services may at its discretion suspend payment of personal leave. In the case of an alleged illness, NRMA may require an employee attend a medical examination by a medical practitioner nominated by NRMA Motoring and Services, in respect of the illness or injury.

  • Physician's Certificate When a female employee applies for pregnancy leave she must provide her supervisor with a certificate from her physician stating that she is pregnant and giving the estimated date of delivery at least two weeks prior to the date she plans to commence the leave. In the case of a female employee who stops working prior to the commencement of her scheduled leave because of a birth, still-birth or miscarriage that happens earlier than the employee was expected to give birth, that employee must, within two weeks of stopping work, give her supervisor: (a) written notice of the date the pregnancy leave began or is to begin, and (b) a certificate from a legally qualified medical practitioner that, (i) states the date of the birth, still-birth or miscarriage and the date the employee was expected to give birth. When a female employee resigns without notifying her supervisor that she is pregnant and she has not applied for pregnancy leave, but within two weeks following her resignation, provides her supervisor with a certificate from her physician stating she was unable to perform her job duties because of a medical condition arising from her pregnancy and giving the estimated or actual delivery date, she shall be entitled to pregnancy leave if it is requested.

  • Distribution of Materials You will keep an accurate record of the names and addresses of all persons to whom you give copies of the Registration Statement, the Prospectus, any Preliminary Prospectus (or any amendment or supplement thereto) or any Offering Circular or any Preliminary Offering Circular and, when furnished with any subsequent amendment to the Registration Statement, any subsequent Prospectus, any subsequent Offering Circular or any memorandum outlining changes in the Registration Statement or any Prospectus or Offering Circular, you will, upon request of the Manager, promptly forward copies thereof to such persons.
