Productivity Standard. Full time home health and hospice nurses shall be 12 required on average to make twenty-five (25) “credited” visits each week, subject 13 to proration for part-time nurses, as listed below. A good faith effort will be made 14 for nurses not to be assigned more than five (5) “credited” visits each day. The 15 Hospital agrees to take extenuating circumstances (e.g. vehicle accident or 16 malfunction, travel delay due to construction or accident, etc.) into account if 17 productivity standards are not being met. 18  Skilled = 1 Visit 19  Non-Admit = 1 Visit
