Proof of Residency. Provide copies of Proof of Residency within the Local 489 geographical jurisdiction for a minimum of 18 months (Pittsburgh). The two required documents to verify residency are the following: a. A valid Driver’s License from Pennsylvania in the jurisdiction of Local 489 AND b. ONE of the following 3 documents: 1. Voter Registration Card 2. A home lease to a property in the jurisdiction 3. Homeownership documents for a property within the jurisdiction AFFLIATED WITH AMERICAN FEDERATION OF LABOR ALLEGHENY COUNTY LABOR COUNCIL International Alliance of Theatrical Stage Employees STUDIO MECHANICS LOCAL 489 IMPORTANT: If you have NOT lived within Local 489 jurisdiction for 18 months, you must request a waiver from the Executive Board in writing with the application. A letter requesting the waiver must accompany your application and must: a. Ask permission to waive the 18 month residency requirement; b. State that your principle residence is the address within Local 489 jurisdiction that appears on your application; and c. Explain your reasons for moving to the jurisdiction and your intentions for staying. The waiver is subject to the approval of the Executive Board PLEASE NOTE: If you have not lived in the jurisdiction for the required 18 months and are submitting a letter requesting the waiver of that requirement, your Local 489 Sponsor MUST attend the Executive Board at which your application is being considered.
Proof of Residency. All taxpayers must submit 2 documents proving residency at the property. Documentation must include the taxpayer’s name and the property address. Acceptable documentation includes: 1) Government-issued ID that has not expired: a. Photo ID issued by the U.S. Federal Government or the Commonwealth of Pennsylvania (including the Department of State Voter ID Card), or b. PA Driver's License or Non-Driver's License Photo ID, or c. U.S. Passport, or d. U.S. Military ID – active duty and retired military (A military or veteran's ID must designate an expiration date or designate that the expiration date is indefinite. Proof of Residency. 8.1 The onus is on the potential successor to provide a sufficient level of evidence. Documents should cover the entire qualifying period. They MUST also provide the death certificate. 8.2 Ekaya will require evidence for ALL succession requests cases to confirm the identity of the person who is claiming succession, their relationship to the deceased, the length of residence with the tenant and where there is a discretionary succession, evidence of their right to rent. 8.3 A potential successor must be in occupation of the tenancy address as their only or principal home. Xxxxx will also conduct other checks to ascertain who was residing in the property in the qualifying period prior to the tenants’ death. 8.4 Where the potential successor fails to provide evidence that they meet the criteria for succession, we will reject their application and seek possession of the property. Pre-action protocol letters would be sent to any occupants.

Any student who transfers from one school to another within the state, without a corresponding change of address, will be ineligible to participate in any VHSL activity for a period of one calendar year from the date of enrollment. If you have lived at this residence for less than 30 days, you will be given temporary enrollment at the time of registration and will have 30 days to provide the required information. Failure to provide required documentation will result in the withdrawal of your child(ren) from school.
