Reporting of Issues. The MCO shall make a good faith effort to promptly report to the STATE any significant delays, errors, deficiencies in service delivery, delays in claims payment to Providers, or failure to implement the correct member materials, including approved templates.

Grievances & Code of Conduct with Other Students: By signing and joining this teacher training, one recognizes that they will be in the company of fellow students and will treat each other with the highest respect. Any grievances must be made known to the Lead Trainer by reporting issues. Grievances will be dealt with on a case-by-case basis, through moderation and inquiry into the nature of the issue. Attendance & Missing Training Hours : Attendance Policy Students are required to maintain a minimum of 95% attendance over the length of the program. All absences must be pre-approved by the instructor and arrangements made to complete the missed amount of classroom time. Failure to make such arrangements may result in termination. If a student must be absent, he/she must call within thirty (30) minutes prior to class start time. There are additional required hours to complete the course through studying, reading, projects and practical hours. You are responsible to make up anything over 10 hours of missed class time through attending workshops by E-RYT’s.

Reporting of Issues bugs Licensee’s designated person will communicate the issue either through any automated Bug reporting system that the Licensor may provide or through an e-mail, the date and time of the mail shall be sufficient for all SLA calculations. All costs associated with resolving the issue reported by the Licensee shall be borne by Licensor unless the issue was caused due to the proven fault of the Licensee. If the issue was caused by the Licensee or should there be a higher level of service required, the Licensee and Licensor shall jointly define the higher level of service and commercial terms. Provided the Licensee is up to date in making all payments under this agreement, the Licensor will be responsible to provide to Licensee in a timely fashion all patches or updates to the Software that it develops along with Documentation explaining the purpose and installation procedures for any such patches or updates and on-going training if and to the extent required. Licensor and Licensee may enter into an agreement for further enhancing the support levels, including providing a person at site.
