Reporting of Issues. The MCO shall make a good faith effort to promptly report to the STATE any significant delays, errors, deficiencies in service delivery, delays in claims payment to Providers, or failure to implement the correct member materials, including approved templates.

I will immediately report to my supervisor or the appropriate Presence Health representative responsible for overseeing the provision of services by me and my company any known or suspected (a) use of my password by someone other than me, or (b) inappropriate access, use or disclosure of CI. By signing and joining this teacher training, one recognizes that they will be in the company of fellow students and will treat each other with the highest respect. Any grievances must be made known to the Lead Trainer by reporting issues. Grievances will be dealt with on a case-by-case basis, through moderation and inquiry into the nature of the issue. Attendance Policy Students are required to maintain a minimum of 95% attendance over the length of the program. All absences must be pre-approved by the instructor and arrangements made to complete the missed amount of classroom time. Failure to make such arrangements may result in termination. If a student must be absent, he/she must call within thirty (30) minutes prior to class start time. There are additional required hours to complete the course through studying, reading, projects and practical hours. You are responsible to make up anything over 10 hours of missed class time through attending workshops by E-RYT’s.
Reporting of Issues. 14.1 The Principal or Contractor shall inform the other party in writing of any issues encountered for proper monitoring and recording for decision making purposes and also for future reference.

All other Severity Level Issues must include a reasonable detailed written description of the decrease in functionality or performance of the Service, and the impact on the efficacy of the Service.
