REQUEST FOR EXTENSION OF LEAVE. Except as provided herein above, to be given consideration for an extension of an approved leave of absence the employee must notify the Director of Labor and Employee Relations or his/her designee not less than twenty (20) work days prior to the expiration of the leave.

REQUEST FOR EXTENSION OF LEAVE. All requests and grants of extension or renewals of leave will be in writing.
REQUEST FOR EXTENSION OF LEAVE. (a) To be given consideration for an extension of an approved leave of absence, the employee must notify the University’s Human Resources Office not less than five (5) working days prior to the expiration of the leave.
REQUEST FOR EXTENSION OF LEAVE. An employee (either parent) entitled to parental leave pursuant to the provisions of clause may request: • to extend the period of unpaid parental leave provided for in this clause by a further continuous period of leave not exceeding 12 months; • to return from a period of parental leave on a part-time basis until the child reaches school age, to assist the employee in reconciling work and parental responsibilities. ANSTO shall consider the request having regard to the employee’s circumstances and, provided the request is genuinely based on the employee’s parental responsibilities, may only refuse the request on reasonable grounds related to the effect on the workplace or the employer’s business. Such grounds might include cost, lack of adequate replacement staff, loss of efficiency and the impact on customer service.

  • Termination of Leave Interruption of pregnancy will terminate the leave. Human Resources may require in such cases forty-five (45) days’ notice to return.

  • Expiration of Leave Except as otherwise provided in this Article, twenty (20) days before the expiration of a leave for ninety (90) days or more, or five (5) days before expiration of a leave for twenty (20) days but less than ninety (90) days, the employee should make every effort to notify the Personnel Office of his/her intention to return, or request an extension of leave, if eligible. Unless such notice is given, failure to return to work upon expiration of the leave may be considered resignation from service.

  • Request for Exemption Grantee may request written approval from Grantor for an exemption from Paragraph 21.2. Grantee acknowledges that Grantor is under no obligation to provide such exemption and that Grantor may, if an exemption is granted, grant such exemption subject to such additional terms and conditions as Grantor may require.

  • Notice of Leave a. The employee shall provide his/her supervisor at least thirty (30)-days prior notice of the need for a leave of absence. If this is not possible, notice shall be given as soon as practicable.

  • Extension of Agreement Prior to the original expiration date of this Agreement, the Parties mutually agree to extend this Agreement to the February 15 extension date identified in Paragraph VIII(A).   • Duration of Leave An eligible female employee may apply for pregnancy leave, to commence after the 22nd week of pregnancy for a duration of up to 17 weeks. The pregnancy leave of an employee who is not entitled to take parental leave ends on the later of the day that is seventeen weeks after the pregnancy leave began or the day that is six weeks after the birth, still-birth or miscarriage.
