Required Referrals. All services rendered by network providers require a network PCP referral except for those services you receive from a network PCP, emergency services, and permitted self-referred services. You are responsible for getting the referral when receiving services from a network provider. If a referral is not received, the service will not be covered. You will be responsible for paying for the billed charges.
Required Referrals i. Contractor shall refer clients to the Screening Assessment and Referral Center if identified by staff that a need for a higher level of substance use disorder treatment service exists. Contractor shall have knowledge of diverse community resources available for basic living needs in order to refer clients for items such as: food, housing, clothing, transportation services and child care services. v. Contractor shall provide referrals, information, literature and pamphlets, etc. in multiple languages and formats as appropriate. vi. Contractor shall initiate diverse collaborative community partnerships and service systems. The Contractor will establish procedures that will ensure strong, reliable linkages with other community service providers, service organizations, Substance Use Disorder treatment network for client support. These collaborative efforts shall be designed to integrate, coordinate and access necessary support services within the community in order to ensure successful client recovery. These efforts shall help achieve mutual goals espoused by Federal, State, and County systems to integrate services, prevent relapse through the use of community support services, reduce fragmentation of care and establish better communication and collaboration at all levels, but particularly among local providers and agencies who work with this target population.

Required Referrals. Public health dental hygienists will refer patients without a dental provider to a public or private dentist with the goal to establish a dental home. • When the dental hygienist determines at a subsequent appointment that there are conditions present which require evaluation for treatment, and the patient has not seen a dentist as referred, the dental hygienist will make every practical or reasonable effort to schedule the patient with a VDH dentist or local private dentist volunteer for an examination, treatment plan and follow up care.
