Tier 1. 4.1.1 Where a medicine supply and any associated pharmaceutical care requirement falls wholly within the scope of the national pharmacy arrangements but the Health Board wishes to direct where the medicine should be sourced through distribution arrangements put in place by the manufacturer that has been contracted to supply the product to the NHS these supplies will attract a Tier 1 engagement fee.
Tier 1. Unit members employed before July 1, 2006 (including disability recipients) are eligible for retiree health and hospitalization plans provided they have completed benefit eligible service of at least 120 months or 10 school calendar years with Elk Grove Unified School District prior to retirement. Retirees/recipients must be actively drawing retirement or disability benefits from the STRS/PERS. Such retirees/recipients shall have met the eligibility requirements during their active employment. The years of benefit eligibility do not have to be consecutive; but if an employee leaves and returns outside of the 39 month rehire period, he or she must again meet the entire vesting requirements in place at that time for benefit eligibility. Eligibility shall be modified upon the receipt of state or federal health and hospitalization coverage; i.e., enrollment in Part A and B of Medicare.
Tier 1. Tier 1 covers Miscellaneous Members hired before March 1, 2015. Overtime shall be computed based upon FLSA standards, with overtime to be paid for all hours worked over forty (40) in a work week. The following shall be counted as hours worked for purposes of computing overtime: vacation time and Compensatory Time Off.
Tier 1. 7.1 The intent of the Tier 1 distribution is to ensure that no one UA is worse off under the pooling system than it would have been without pooling on a year by year basis. The formula below is how the Tier 1 payment will be calculated only if the UA has triggered the application of the authority levy under the National Resource System in that year. Tier 1 payments will otherwise be calculated excluding the application of Authority Levy element in the formula below.
Tier 1. Certain current or former Class Representatives who testified at a deposition in the Action and who attended one or more settlement conferences in their capacity as class representatives—Xxxxxxx Xxxxxx and Xxxxx Xxxxxxx—shall each be paid $2,500 as an Incentive Award.
Tier 1. Retiree Medical for Employees Hired Prior to March 31, 2008 Employees hired prior to March 31, 2008 who retire prior to January 1, 2015 with a minimum of 5 years of service with the City will receive a retiree medical benefit equivalent to the amount necessary for actual enrollment cost in single, two-party, or family coverage, up to a maximum dollar amount of the CalPERS Bay Area Kaiser family premium rate. Retirees must be enrolled in a CalPERS plan in order to receive the retiree medical benefit.
Tier 1. Students in Tier 1 can access one course at a time. Registration into an additional course may occur once the preceding course grade has been finalized and awarded.
Tier 1. The City shall maintain California Public Employees’ Retirement System (CalPERS) benefits for Unit employees who are deemed to be “classic members” and who are employed with the City in a miscellaneous classification on or prior to March 2, 2012. The CalPERS benefit shall be based upon the 2% @ 55 retirement benefit formula, calculated using the single-highest year final compensation period (Govt. Code 20042).
Tier 1. The Tier 1 Payment shall be made available through a Voucher code sent automatically by the Settlement Administrator as part of Class Notice. Each Settlement Class Member will be able to enter the Voucher code online from the time of receipt up through the Tier 1 Claims Submission Deadline, through a settlement website created by the Settlement Administrator. The Tier 1 Payment shall be a One Hundred Twenty-Five Dollar ($125) Starbucks Gift Card, which may take the form, at Starbucks’ discretion, of a physical card or a QR code that may be redeemed at Starbuck’s stores for a gift card. Gift Cards shall have no expiration date and shall be freely transferrable. The Settlement Administrator shall maintain a registry of Class Members for a period of no less than twelve (12) months after the Effective Date in which to provide Class Members without email addresses or invalid email addresses the Voucher code for the Gift Card. 4.03.02 Tier 2. The Tier 2 Payment is made through the payment of either: (1) an additional One Hundred Twenty-Five Dollar ($125) Starbucks Gift Card; or (2) a check for $90, at the election of a Tier 2 Claimant. Tier 2 relief is only available for Class Members who answer yes to all of the statements contained in a Claim Form for Tier 2 Claimants, which will be substantially the same as the Tier 2 Claim Form attached hereto as Exhibit E. To be considered, all Claim Forms must be complete, signed by the Tier 2 Claimant under penalty of perjury, and submitted by no later than the Tier 2 Claims Submission Deadline, as set forth in Paragraph 4.04.
