Resident Representative. The Resident Representative is the individual designated to receive information and assist and/or act on behalf of the Resident to the extent permitted by State law. Unless the Resident Representative is also the Resident's spouse or Sponsor, the Resident Representative is not obligated to pay for the cost of the Resident's care from his/her own funds. Notwithstanding the foregoing, to the extent the Resident Representative breaches the obligations personally undertaken to ensure that the Resident has a payment source for his/her nursing home care (either from private funds and/or a third party payor) he/she may be personally liable to the Facility for the damages caused by said breach. By signing this Agreement, the Resident Representative hereby represents and warrants that he/ she shall (i) utilize the Resident’s funds to pay for the Resident’s care at the Facility to the extent he/she has access to such funds; (ii) timely provide information and documentation requested by the Facility or a third party payor including, but not limited to, insurance and/or Medicaid; and (iii) timely provide accurate and complete information and documentation to the Facility regarding such matters as the Resident’s financial resources, citizenship or immigration status and third party insurance coverage. The Resident Representative xxxxxx agrees to indemnify and hold the Facility harmless from any loss, damage or expense the Facility may suffer or incur as a result of a breach of the foregoing representations and warranties. The Resident Representative acknowledges that nothing herein constitutes an impermissible third party guarantee of payment; rather, this Agreement sets forth independent obligations that are being voluntarily undertaken by the Resident Representative. The provisions in this paragraph shall survive termination of this Agreement for any reason.

Resident Representative. Within thirty (30) days after the Effective Date Contractor shall designate a representative residing in Trinidad and Tobago who shall have full authority to represent it in respect of matters related to the Contract and to receive notices addressed to Contractor.

Resident Representative. Each Founding Shareholder shall ensure that at least one (1) senior employee of that Founding Shareholder with authority to make representations on behalf of, and take any actions delegated to him by, such Founding Shareholder, is resident in The Kingdom during the Term.
Resident Representative. If applicable, this Agreement shall be signed by the person legally responsible for making health care decisions on behalf of Resident (e.g., health care power of attorney or guardian of the person) and any person who has legal access to the income and/or financial resources of Resident (e.g., financial power of attorney or guardian of the estate) (collectively “Resident Representative”). The term “Resident” as used in this Agreement shall be construed to mean “Resident and Resident Representative” unless the context does not so permit.
Resident Representative. The resident’s representative, if applicable, is an adult making decisions on behalf of the resident. Has the resident been appointed a legal guardian through a court, or is an adult appointed by the resident acting as POA due to the resident’s current incapacitation? Yes No If “Yes” above, the provider will ensure the representative’s information is reflected in the admission records, including maintaining a copy of the document confirming the representative’s appointment in the resident’s records.
Resident Representative. The resident representative of the Partnership shall be Xxxx Xxxxxx, 00 Xxxxx Xxxxxx, Xxxxxxxx XX00, Bermuda, or such other person as may be appointed by the General Partner.
Resident Representative. 5.1. Resident Representative means any of the following: (1) An individual chosen by the Resident to act on behalf of the Resident in order to support the Resident in decision-making; access medical, social or other personal information of the resident; manage financial matters; or receive notifications; (2) A person authorized by State or Federal law (including but not limited to agents under power of attorney, representative payees, and other fiduciaries) to act on behalf of the Resident in order to support the Resident in decision-making; access medical, social or other personal information of the resident; manage financial matters; or receive notifications; (3) Legal representative, as used in section 712 of the Older Americans Act; or (4) The court- appointed guardian or conservator of a resident.
5.2. The Resident Representative represents and warrants that he or she has legal access to your income, assets and resources to pay for the services provided by us.
5.3. The Resident Representative shall execute the Resident Representative Agreement attached hereto at Exhibit C.
5.4. The Resident Representative does not, by reason of signing this Agreement, assume an obligation to reimburse us for your care, except from your income, assets and resources; however, the Resident Representative does assume other legal obligations as set forth in this Agreement and the Resident Representative Agreement and may be held legally responsible for failing to fulfill these obligations.
5.5. The Resident Representative shall provide us with his or her current home address, e-mail, and phone upon your admission and in the event of any change. If the Resident Representative fails to provide us with his or her current home address and phone, you and the Resident Representative waive any right the Resident Representative would otherwise have had to any notices.
5.6. If the Resident Representative executes this Agreement on the Resident's behalf, the Resident Representative represents that he or she is authorized to do so.
