(d) Names of students. Items (a) and (b), if provided to SpeechGear, will be used to estimate the initial number of rooms assigned to each building along with the creation of a building specific admin account. Items (c) and (d), if provided to SpeechGear, will be used to create Streamer™ accounts that students and staff will use to access the captioning, note-taking and translation system. Again, providing this data to SpeechGear is NOT required. This is completely optional. School District personnel can create these accounts internally. SpeechGear is simply offering as a free service to help with this process.

  • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.

  • Chiropractic Services This plan covers chiropractic visits up to the benefit limit shown in the Summary of Medical Benefits. The benefit limit applies to any visit for the purposes of chiropractic treatment or diagnosis.

  • Health Care Operations “Health Care Operations” shall have the same meaning as the term “health care operations” in 45 CFR §164.501.

  • Hospice Services Services are available for a Member whose Attending Physician has determined the Member's illness will result in a remaining life span of six months or less.
