Services and Service Requirements. 4.2.1 The Parties agree to work together in accordance with the terms of this Agreement to enable Students to work towards a UCL Awarded Degree incorporating a Placement at the Practice. The Practice shall deliver the Services and meet the stipulated quality and performance requirements to be delivered under this Agreement in accordance with the provisions of this Agreement, including Schedule A of this Agreement. 4.2.2 Students allocated a Placement must be registered Students of the University working towards a UCL Awarded Degree. 4.2.3 The number of Students undertaking a Placement will be agreed in collaboration with the Practice according to the processes of Placement organisation set out in Section 2 of Schedule A of this Agreement. 4.2.4 The University shall perform its obligations under this Agreement and the Practice shall deliver the Services in accordance with (i) the terms and conditions of this Agreement

Services and Service Requirements. Client eligibility requirements for this program are as follows: • Any person requesting one or more of the services listed below is considered a family planning client regardless of funding sources; • Uninsured clients less than 100% of the Federal Poverty Level (FPL) qualify for free services; and • Uninsured clients between 101% - 250% of the FPL qualify for subsidized services based on a sliding fee schedule. Services to be provided by the PROVIDER will include: • Condoms and will refer clients that want a wide range of FDA approved contraceptive methods to a local Title X site; • Pregnancy testing and counseling; • Referral to a local Title X site for clients who wants services to conceive • Referral to a local Title X site for clients who want basic infertility services; • Referral to a local Title X site for clients who want preconception health services; • Sexually transmitted disease screening and treatment and; • Education and counseling. PROVIDER must be in full compliance with Section 1008 of the Title X statute and 42 CFR 59.5 (a) (5), which prohibits abortion as a method of family planning. PROVIDER will be responsible for providing the Family Planning Program in the following counties: York County PROVIDER will ensure that every service site is in compliance with the following policies and procedures as delineated in: • FHCCP Family Planning Manual; • OPA’s Program Requirements for Title X Funded Family Planning Projects; • CDC’s MMWR on Providing Quality Family Planning Services; • CDC’s U.S. Selected Practice Recommendations for Contraceptive Use; • CDC’s U.S. Medical Eligibility Criteria for Contraceptive Use; • CDC’s Revised Recommendations for HIV Testing of Adults, Adolescents, and Pregnant Women in Health-Care Settings; • CDC’s 2015 Sexually Transmitted Diseases Treatment Guidelines; and • CDC’s Recommendations to Improve Preconception Health and Health CareUnited States. PROVIDER shall identify and collaborate with other service providers and individuals in their community to connect clients to services that fall outside the scope of the Family Planning Program. At a minimum, written collaborative agreements with relevant referral sources exist including: emergency care, HIV/AIDS care and treatment, infertility specialists, primary care and chronic disease management.

  • Interface Requirements 2.4.5.1 The NID shall be equal to or better than all of the requirements for NIDs set forth in the applicable industry standard technical references.

  • SPECIALIZED SERVICE REQUIREMENTS In the event that the Participating Entity requires service or specialized performance requirements not addressed in this Contract (such as e- commerce specifications, specialized delivery requirements, or other specifications and requirements), the Participating Entity and the Supplier may enter into a separate, standalone agreement, apart from this Contract. Sourcewell, including its agents and employees, will not be made a party to a claim for breach of such agreement.

  • Forecasting Requirements 19.5.1 The Parties shall exchange technical descriptions and forecasts of their Interconnection and traffic requirements in sufficient detail necessary to establish the Interconnections necessary for traffic completion to and from all Customers in their respective designated service areas.

  • Invoice Requirements Contractor hereby waives the right to enforce any term which contradicts or modifies any term of the solicitation or any Contract that may result, including subsequent amendments to the Contract, or would result in an unencumbered expense if enforced against the state. Contract quote and invoice must contain, at a minimum:

  • Subcontractor Requirements The Supplier must ensure that any subcontract entered into for the purpose of this Agreement contains an equivalent clause granting the rights specified in this clause.

  • Performance Requirements A. There is no guaranteed minimum amount of work which will be ordered under this Contract. B. The total Contract amount will not exceed $4,900,000. C. This is a Contract for work specified in individual Job Orders. Work ordered prior to but not completed by the expiration of the Contract period, and any additional work required as a result of unforeseen conditions encountered during construction up to six (6) months after the contract expiration date, will be completed with all provisions of this Contract still in force. Performance time for each Job Order issued under this Contract will be determined in accordance with the Contract. This performance time will be determined and agreed upon by both Parties for each individual Job Order. Contractor must self-perform 20% of the Work under this Contract for ‘A’ and ‘B’ licenses. Contractor must self-perform 75% of the Work under this Contract, unless otherwise approved by the County, for ‘C’ licenses. D. This is an indefinite-quantity Contract for the supplies or services specified and effective for the period stated. Work or performance shall be made only as authorized by Job Orders issued in accordance with the ordering procedures clause. The Contractor agrees to furnish to the County when and if ordered, the supplies or services specified in the Contract up to and including the quantity designated in the Job Orders issued as the maximum designated in the Contract.
