Services to Students with. Disabilities The DSBPC is responsible for providing and funding reasonable accommodations for qualified students with documented disabilities (e.g. Transition Individual Educational Plan (TIEP), 504 Plan), who are enrolled in a PHSC dual enrollment course(s) during regular high school hours on the high school campus(es). Students with disabilities who are enrolled in dual enrollment courses offered at PHSC sponsored sites (campuses, centers, etc.) will receive accommodations upon receipt of documentation of a disability(ies) that meets PHSC guidelines. The DSBPC and PHSC will share equally in the cost of reasonable accommodations that are mutually agreed upon for dually enrolled students. Any costs for students with hearing impairments needing sign language interpreters or students with visual impairments who take classes at a PHSC campus will be paid 50% by DSBPC and 50% by PHSC. Nothing herein shall make PHSC a party to the TIEP nor shall PHSC be given or required to have a TIEP with a dual enrollment student. PHSC's Office of Disabilities Services shall work with the DSBPC to determine accommodations required under the Americans with Disabilities Act and Section 504. In order to receive accommodations, students must self-disclose their disability. Information regarding eligibility for instructional and support services and documentation of a student’s disability that is required before providing accommodations for each disability by submitting the required form or by contacting the Cooridnator of Disabilities Services on PHSC’s West Campus. Determination of eligibility is the responsibility of PHSC’s Coordinator of Disabilities Services. PHSC is an equal access college and does not discriminate against person with disabilities. PHSC, in compliance with Section 504 of the Rehabilitation Act, the Americans with Disabilities Act (ADA) and ADA Amendment Act, makes every reasonable effort to accommodate persons with disabilities. Students requiring any special services or accommodations because of a disability should contact the coordinator of disabilities services at the West Campus or the assistant coordinator of disabilities services at any other campus, and complete a Self-Identification of Disability form. The College requires documentation of a student’s disability before providing accommodations for such disability.   • Pharmacy Services The Contractor shall establish a network of pharmacies. The Contractor or its PBM must provide at least two (2) pharmacy providers within thirty (30) miles or thirty (30) minutes from a member’s residence in each county, as well as at least two (2) durable medical equipment providers in each county or contiguous county.

  • Diagnostic Services Procedures ordered by a recognized Provider because of specific symptoms to diagnose a specific condition or disease. Some examples include, but are not limited to:

  • Medical Services Plan 10.1.1 Regular Full-Time and Temporary Full-Time Employees shall be entitled to be covered under the Medical Services Plan commencing the first day of the calendar month following the date of employment.

  • Related Services Licensee shall be responsible for obtaining and installing all proper hardware and support software (including operating systems) and for proper installation and implementation of and training concerning the Licensed Software. In the event that Licensee retains Licensor to perform any services with respect to the Licensed Software (for example: installation, implementation, maintenance, consulting and/or training services), Licensee and Licensor agree that such services shall be subject to Licensor’s then current standard terms, conditions and rates for such services unless otherwise agreed in writing by Licensor.

  • Project Management Services Contractor shall provide business analysis and project management services necessary to ensure technical projects successfully meet the objectives for which they were undertaken. Following are characteristics of this Service:

  • Educational Services Any service or supply for education, training or retraining services or testing including: special education, remedial education; cognitive remediation; wilderness/outdoor treatment, therapy or adventure programs (whether or not the program is part of a Residential Treatment facility or otherwise licensed institution); job training or job hardening programs; educational services and schooling or any such related or similar program including therapeutic programs within a school setting.

  • Telemedicine Services This plan covers clinically appropriate telemedicine services when the service is provided via remote access through an on-line service or other interactive audio and video telecommunications system in accordance with R.I. General Law § 27-81-1. Clinically appropriate telemedicine services may be obtained from a network provider, and from our designated telemedicine service provider. When you seek telemedicine services from our designated telemedicine service provider, the amount you pay is listed in the Summary of Medical Benefits. When you receive a covered healthcare service from a network provider via remote access, the amount you pay depends on the covered healthcare service you receive, as indicated in the Summary of Medical Benefits.   • Technical Services Party B will provide technical services and training to Party A, taking advantage of Party B’s advanced network, website and multimedia technologies to improve Party A’s system integration. Such technical services shall include:

  • Development Services During the term of this Agreement, the Provider agrees to provide to or on behalf of the Port the professional services and related items described in Exhibit A (collectively, the “Development Services”) in accordance with the terms and conditions of this Agreement.
