Settlement Benefits to Class Members. 58. Each Class Member who does not submit a Valid Exclusion Statement and submits a Valid Claim Form in accordance with Paragraphs 96-97 of this Settlement Agreement is eligible to choose one, but not both, of the following types of Participating Class Member Payments:

Subject to the caps described below, Settlement Class Members may submit a claim to receive the following benefits: (1) Credit Monitoring Services; (2) payment for Unreimbursed Economic Losses; (3) payment for Lost Time; or, in the alternative to benefits (1)-(3), an Alternative Cash Payment of $100.00.
Settlement Benefits to Class Members. Settlement Class Members seeking reimbursement under ¶ 2.3 must complete and submit a Claim Form to the Claims Administrator, postmarked or submitted online on or before the 90th day after the date on which the notice program commences — the “Claims Deadline.” The notice to the Settlement Class will specify this deadline and other relevant dates described herein. The Claim Form must be verified by the Settlement Class Member with a statement that his or her claim is true and correct, to the best of his or her knowledge and belief. Notarization shall not be required. Failure to provide supporting documentation for Ordinary Losses (other than for reimbursement for lost time) and Extraordinary Losses, as requested on the Claim Form, shall result in denial of a claim. For reimbursement of lost time claimed by Settlement Class Members, the Settlement Class Member must provide an attestation that the time claimed was spent dealing with issues related to the Data Breach.
