Statement of Qualifications Requirements. This curriculum must consist of Pre-ETS required activity and must be approved by MDRS prior to the start of services. (These core requirements are listed under each required activity) Providers will allow students to make-up missed modules when it is offered next. Each Pre-ETS topic area (e.g. work readiness) will begin and conclude with a Pre/posttest which will be given to the MDRS counselor as documentation of progress after the completion of all authorized activities. A completed Pre-ETS Provision form is required for each student with the dates of service of the Pre-ETS activities provided and is to be returned to the Transition Counselor monthly until the services are completed. Invoicing and Client/Participant Report: All services must be pre-authorized by MDRS according to the established fee schedule. Invoices must include authorization number/purchase order number, the client’s/participant’s name and the amount of time billed for his/her involvement in each Pre-ET activity, a description of the services provided, service start and end dates, and the appropriate service fee. Providers will not bill Students, parents, or the LEA for Pre-ET services. Invoices for Pre-ETS training (which includes: Job Exploration Counseling, Counseling on Opportunities for Enrollment in Comprehensive Transition or Postsecondary Educational Programs at Institutions of Higher Education, Workplace Readiness Training and Instruction in Self-Advocacy) must be submitted with the following mandatory documentation: Pre-ETS Provision form with a summary of the activity and Pre/Posttests. Detailed Invoices for each student authorization and the amount of time billed for his/her involvement in each Pre-ETS activity. Reports must be submitted monthly until all the services are provided. Services provided without prior authorization will not be paid by MDRS. Payment may be delayed or denied if the invoice is not submitted correctly and/or the required reports are not submitted; providers will not receive payment for claims submitted after 90 days from the date of the service. Travel cost has been calculated into the unit rates and will not be paid separately by MDRS to Contract providers.

  • Vendor Qualifications Minimum Qualifications Based on the Contractor’s submission, OGS has determined that Contractor met the minimum qualifications for the lot(s) as identified in Appendix G, Contractor and OGS Information. If Contractor is awarded Lot 1 as a New York State Certified Minority- or Women- Owned Business Enterprise or as a New York State Small Business, Contractor agrees that it must retain at least one of such statuses to retain its Lot 1 award. Should a Contractor no longer retain at least one of such statuses, OGS shall suspend its Lot 1 award and the Contractor shall not be able to respond to Authorized User’s requests. If the Contractor fails to regain at least one of the statuses within 90 calendar days and provide OGS with documentation of such status, then its Lot 1 award shall be terminated.   • Required Qualifications Not Applicable

  • General Qualifications General qualifications commonly required of all candidates for, appointees to, and employees in the classified service such as integrity, honesty, sobriety, dependability, industry, thoroughness, accuracy, good judgment, initiative, resourcefulness, courtesy, ability to work cooperatively with others, good health, and freedom from disabling defects, shall be deemed to be a part of the personal characteristics of the minimum qualifications of each class specification and need not be specifically set forth therein. The Commission may prescribe alternative or additional qualifications for individual classes and such qualifications also shall be deemed to be a part of the specifications for such classes.

  • Contractor Qualifications Contractor warrants that Contractor has the necessary licenses, experience and technical skills to provide services under this Contract.

  • Promotions Requiring Higher Qualifications Consideration for promotion shall be given to the senior applicant who does not possess the required qualifications but is presently obtaining such qualifications prior to filling the vacancy. At the discretion of the Employer, such an employee may be given a trial period to qualify within a reasonable length of time and will revert to her former position if the required qualifications are not met within such time.
