SUBMITTAL FORMS. A. The following forms attached to this Part 4 are to be used for submittal purposes: 1. The Submittal Transmittal form is to be used when tendering submittals unless another transmittal form is stipulated. 2. The P.E. Certification form is to be used for documenting professional registration and compliance with Specifications when a particular submittal requires Seller's design to be performed by an engineering professional. 3. The Equipment Record Form. 4. The Manufacturer's Installation Certification Form. B. A digital version of these forms can be obtained upon request to Seller.
SUBMITTAL FORMS a. Certificate of insurance. b. Statement and explanation of any instances regarding past governmental agency bidding or contract disqualifications or removal from a project. After an acceptable agreement is negotiated with the City, the agreement will be executed by its Public Services Director. Upon receipt of a properly executed agreement, a request for a detailed scope of services will be requested and notification to start services will be issued. It is expected that the consultant will commence services within one week after the agreement is executed.

  • Submittal Schedule Within sixty days after the Effective Date of the Contract, the Contractor shall prepare and submit a Submittal Schedule for review and approval of the Design Professional. In establishing the Submittal Schedule the Contractor shall take into account large submittal documents that will require longer review times, e.g., submittals with over fifty sheets of drawings. The Design Professional’s approval shall be based on conformance of the Submittal Schedule with the Overall Project Schedule, subject to change from time to time in accordance with the progress of the Work.

  • ELECTRONIC SUBMITTALS Contractor shall obtain a license for the State to utilize Submittal Exchange for the purposes of this project. The State and its representatives will have full control of the use of Submittal Exchange by authorized users of the State.
