Common use of Vision Care Plan Clause in Contracts

Vision Care Plan. The Company will provide a Vision Care Plan which will provide for expenses incurred by an employee and/or the employee's covered dependants when recommended by a physician or optometrist as follows: - frames, lenses, and the fitting of prescription glasses including contact lenses, up to a total of one hundred and eighty dollars ($180.00) per family member, in any two (2) consecutive calendar years.

Effective September 1, 2004, the maximum payment will be increase to One Hundred and Sixty Dollars ($160.00).
