Continuity of Care Document definition

Continuity of Care Document means an electronic file that summarizes an Individual’s medical status in a standard format for the purpose of information exchange. It contains a core data set of the most relevant administrative, demographic, and clinical information about an Individual's health care, covering one or more health care encounters.
