Proof of Claim Form definition

Proof of Claim Form means the form, substantially in the form attached hereto as Exhibit 2 to Exhibit A, that a Claimant must complete and submit should that Claimant seek to share in a distribution of the Net Settlement Fund.
Proof of Claim Form means the form provided to Class Members by the Claims Administrator for purposes of submitting a Claim, substantially in the form attached hereto as Exhibit A-3.

More Definitions of Proof of Claim Form

Proof of Claim Form means the form, substantially in the form attached hereto as Exhibit 2 to Exhibit A, that a Claimant must complete and submit to the Claims Administrator in order to be eligible to share in a distribution of the Net Settlement Fund.
Proof of Claim Form or “Claim Form” means the proof of claim form and release to be mailed to Settlement Class Members, as approved by the Court, which Settlement Class Members shall be required to complete and return to the Claims Administrator in order to substantiate their entitlement to a share of the Net Settlement Fund, and which shall be substantially in the form attached hereto as Exhibit A-2.
