Proof of Claim Form definition

Proof of Claim Form means the form, substantially in the form attached hereto as Exhibit 2 to Exhibit A, that a Claimant must complete and submit should that Claimant seek to share in a distribution of the Net Settlement Fund.
Proof of Claim Form means the form provided to Class Members by the Claims Administrator for purposes of submitting a Claim, substantially in the form attached hereto as Exhibit A-3.
Proof of Claim Form means the form, substantially in the form attached hereto as Exhibit 3 to Exhibit A, that a Class Member must complete should that Class Member seek to share in a distribution of the Net Settlement Fund, subject to entry of a Plan of Allocation and a Class Distribution Order that have both become Final.

More Definitions of Proof of Claim Form

Proof of Claim Form means the form, substantially in the form attached hereto as Exhibit 2 to Exhibit A, that a Claimant must complete and submit to the Claims Administrator in order to be eligible to share in a distribution of the Net Settlement Fund.
Proof of Claim Form or “Claim Form” means the proof of claim form and release to be mailed to Settlement Class Members, as approved by the Court, which Settlement Class Members shall be required to complete and return to the Claims Administrator in order to substantiate their entitlement to a share of the Net Settlement Fund, and which shall be substantially in the form attached hereto as Exhibit A-2.
Proof of Claim Form means the Proof of Claim and Release form for submitting a Claim to be approved by the Court, which shall be substantially in the form attached hereto as Exhibit A-2, that a Settlement Class Member must complete and submit should that Settlement Class Member seek to share in a distribution of the Net Settlement Fund.
Proof of Claim Form means the Proof of Claim and Release Form required to be used in order to make a claim for payment under this settlement. A copy of the proposed Proof of Claim Form is attached as Exhibit C. The pre-prepared Claim forms shall be bar coded to link with the Class member’s database information and shall contain the damages to which the class member is entitled if s/he were to receive the full damages provided for in this agreement.
Proof of Claim Form means the form designed by the Fund Administrator in accordance with the terms of the Plan for the filing of a claim, and approved by Commission staff, which form shall require, at a minimum, sufficient documentation of all claimed transactions. Proof of Claim Forms will be available on the website established in connection with the Fair Fund. The Claim Form (whether submitted in hard copy or online through the Settlement Website or by email) will require each Class Member to attest under oath that he or she meets the Eligibility Requirements and to submit the Required Documentation to prove their entitlement to compensation, if any, from the Settlement Fund.
Proof of Claim Form means the form that Settlement Class Members may submit to obtain compensation under this Settlement.
