Usual and Customary Retail Price definition

Usual and Customary Retail Price means the Provider's usual and customary retail price of a Covered Medication in a cash transaction (in the quantity dispensed) on the date that it is dispensed, including any discounts or special promotions offered on such date.
Usual and Customary Retail Price means the price that the pharmacy or Provider would charge a customer in the absence of the Card Program.
Usual and Customary Retail Price means the total price charged to a consumer who does not have prescription drug coverage under a health benefit plan.

  • Eligible business means a business located within the counties of Cook, DuPage, Kane, Lake, McHenry or Will in the State of Illinois (the “Six County Region”), and as to which: (1) a majority of the business' fleet is located and used within the Six County Region; and (2) a majority of those vehicles located and used within the Six County Region are alternatively powered vehicles.

  • Finished Services means complete end-to-end services offered by CenturyLink to wholesale customers or retail End User Customers. Finished Services do not include Unbundled Network Elements or combinations of Unbundled Network Elements. Finished Services include Access Services, private lines, retail services, and resold services.

  • Reasonable and Customary Charges means the charges for services or supplies, which are the standard charges for the specific provider and consistent with the prevailing charges in the geographical area for identical or similar services, taking into account the nature of the illness / injury involved.

  • Semi-annual (2/Year) sampling frequency means the sampling shall be done during the months of June and December, unless specifically identified otherwise.

  • Eligible Services means only those services detailed in the sub-section titled Eligible Services.
