Advance notice requirement. The employee must provide the Employer with thirty (30) days written advance notice of leave, or such notice as is practicable, if thirty (30) days notice is not possible. The request for Family Medical Leave should be submitted on the appropriate form to the Human Resources Administrator. In unexpected or unforeseeable situations, an employee should provide as much notice as is practicable, usually verbal notice within one or two business days of when the need for leave becomes known, followed up by written form submitted within a reasonable amount of time. Any items not addressed in this contract shall be addressed by the Personnel Policies Manual, Section 10.6, Family Medical Leave Act.
