Aftercare. 5.1 Contractor shall provide aftercare services for up to 3 months following closure of the Family Skills Development Training program. Services shall be provided at least twice per month on a face to face basis with the juvenile and the family. During the First month of aftercare services, the Contractor shall provide two (2) face to face contacts with the family. During the second and third months of aftercare services, the contractor shall provide one (1) face to face contact and one
(1) telephone contact. If it is jointly determined by the Contractor and the assigned Juvenile Probation Officer that additional face to face contacts are needed, then Contractor shall provide an agreed upon number of face to face contacts.
5.2 Contractor shall monitor the progress of treatment and skills acquired while participant was engaged in the program, as well as ensure appropriate linkage to community based agencies and resources.
5.3 Contractor shall provide support and continued linkage to necessary services in the community.
Aftercare. Aftercare" means care, treatment and services provided a patient, as defined, on convalescent status or conditional release. [ 1983, c. 459, §7 (NEW) .]
Aftercare. 2.4(1) Duration of aftercare will be up to 3 months following closure of the intensive in- home program. Services shall be provided at least twice per month with the therapist providing at least one face-to-face contact.
2.4(2) Aftercare will include monitoring of the internalization of treatment and skills acquired while participant was engaged in the program.
2.4(3) Contractor will provide support and continued linkage to necessary services in the community.
Aftercare. Aftercare is billed with your monthly tuition statement. Charges are due the 1st of the month with your tuition. Any Aftercare account that becomes 30 days late, the child will not be allowed to participate in our Aftercare Program until that account becomes current.
Aftercare. After booking a trip, people get an email confirmation. The people who use email find this reassuring: I use email and like the service where I receive a confirmation by email. I'm glad to see it's okay then (FP4, Ap126-127). FP2, FP6, and FP7 also appreciate the confirmation via email. “For example, I once saw that I had forgotten a reservation. This provides additional confirmation and security.” (FP2, 112). . “My husband appreciates the email confirmation. Suppose this would be via text message then I would also succeed.” (FP6, Ap144). “I find the email confirmation easy.” (FP7, Ap151).
Aftercare a. The RHY Providers will provide case management and follow-up services to Pilot Participants who successfully obtain a CityFHEPS unit for up to six
Aftercare a. HRA will make resources such as Homebase available to Pilot Participants.
b. HRA will also process landlord applications to the Special Supplemental Assistance fund, which provides up to $3,000 for repairs of documented participant damages or unpaid rent after Participants have left the unit.
Aftercare. The Contractor shall provide a comprehensive aftercare component that supports a child's transition into and stabilization within their biological, relative, or an adoptive home. Prior to discharge, the Contractor will provide DCF and the family with an individualized Aftercare Support Services Plan developed in collaboration with DCF. The plan will be flexible and tailored to the needs of the child and family to be served. At a minimum, the Contractor will be available in a "consultative" role as a supportive resource for families/caregivers or subsequent service providers. The Contractor‟s Aftercare Support Services Plan will include, but will not be limited to, the following components:
i. aftercare goals and objectives, as outlined in the TFC Care Plan;
ii. menu of aftercare services and referrals provided;
iii. number and frequency of family contacts; detailing family and child transition progress;
iv. behavior management strategies, and crisis support
v. coordination and facilitation with local community services that will further promote successful transitions;
vi. data tracking and performance measures to monitor success of aftercare services. Aftercare services may be provided to a family for up to 6 months, but must be provided for no less than 4 weeks after discharge, including a minimum of one face to face visit to the child and family and two phone calls. TFC 3 As taken from the Programs and Standards for Treatment Xxxxxx Care, Xxxxxx Family-based Treatment Association (1995) agencies may include a child‟s xxxxxx parent as part of the after-care support team as a means to assist with continuity of care. (e.g., serve as a coach and model to encourage, empower and support the birth or adoptive family or serve as a respite resource to the biological or adoptive family). Aftercare services would also be similarly used to support children who are transitioning from therapeutic xxxxxx care to an adoptive or relative home. The Department may, at its discretion, enter into PPSP contracts with the Contractor to support extensive and longer term aftercare services that are congruent with the child's permanency plan.
Aftercare. A. Contractor agrees to provide formal aftercare services for all youth placed in the 44 bed post-adjudication program. Aftercare services shall be as described in Contractor's proposal, which is included as Exhibit B to this contract. Aftercare services will include, but are not limited to the following program components:
1. Intensive pre-release planning; 2. Family involvement, including in-home visits; 3. Community linkage; 4. Facilitating the process of r~ -enrolling youth in home school or accessing GED assistance services;
5. Assisting age appropriate youth in obtaining employment; and 6. Developing plans for leisure time activities.
B. Contractor shall primarily utilize existing community-based resources in developing aftercare plans. Contractor may, when community-based services are not available, request that the Juvenile Department consider placement of youth in contract programs. Juvenile Department shall review these requests on a case-by-case basis. The decision to place youth in contract programs will be made at the sole discretion of Juvenile Department.
C. Contractor will conduct formal follow-up studies of juveniles who successfully discharge the post-adjudication program and who reside within Dallas County at one month, two months and three months to document successful re-entry into the community, and will track status of critical outcome areas including education, employment, relationships with parents, peers and others, and positive living arrangements.
D. Contractor will provide information gathered in the follow-up studies to the designated Juvenile Department staff.
Aftercare. 11.1 You will receive aftercare instructions and counselling from your surgeon or hospital. The hospital and surgeon will not assume responsibility if you do not follow these instructions and there are consequences as a result of your lack of care.
11.2 Your GP/family doctor should be your initial point of contact after you return to your home country. We will assist you in providing a report from your GP, images, test results, and other information to the surgeon in order for the surgeon to provide an opinion if they find a post- operative care concern with your surgery or indicators of any complications.
