Certification for Leave. A medical certificate, paid for by the employer shall be required;
i) for any sick leave of absence of three (3) consecutive days or longer;
ii) for cumulative sick leave in excess of five (5) days usage in a calendar year which has not been medically certified
iii) for any sick leave of absence if abuse is suspected; Medical certificates, when required, shall be provided within three (3) working days of the commencement of the leave.
Certification for Leave. A medical certificate may be required under the short-term plan:
(i) For any sick leave of absence of three consecutive days or longer.
(ii) For cumulative sick leave usage in excess of five (5) days in a calendar year, which has not been medically certified.
(iii) Where the Employer has reason to suspect abuse of sick leave.
(iv) At the discretion of the Employer during the period of short term sick leave if the leave continues beyond three consecutive days. An employee shall be given notice of the requirement for a medical certificate prior to the return to work. Medical certificates, when required, should be provided upon return to work but in any event, not later than five (5) working days after the return to work. Costs of such certificates shall be paid by the Employer.

The employee must respond to such a request within 15 days of the request or provide a reasonable explanation for the delay. Failure to provide certification may result in a denial of continuation of leave.
