Client Fees. Other than as provided for in the Annex (es) and/or Departmental Component specific policies, the Provider Agency shall impose no fees or any other types of charges of any kind upon recipients of Contract services.

Client Fees. 1. The maximum client fee for each level of service and related fees for Driving Under the Influence (DUI) programs shall be fixed by the County Alcohol and Drug Services Administrator subject to approval by the State Department of Alcohol and Drug Programs.
2. The maximum client fee for the Deferred Entry of Judgment (DEJ) programs shall be fixed by the County Alcohol and Drug Services Administrator subject to the approval of the County Board of Supervisors.
3. The maximum client fee for the DEJ Education Sanction program shall be fixed by the County Alcohol and Drug Services Administrator.

Client Fees. 1. Client acknowledges and agrees that the initial registration fee during this promotional period only is $0 per account. An account constitutes any family unit, whether it be one person or multiple people. All members of the account must be immediate, first degree relatives (e.g. father, mother, sister, brother) living in one household. Individuals who are not immediate family, are a dependent over 18 years old and living in the household, and/or live in another household must be set up on a separate account. Client must be prepared to verify age and relationship status and provide government issued identification at sign up. A membership card will be provided to the client to be used for office visits and to qualify for discounts at participating facilities. This membership card is property of 7 Health and must be surrendered to 7 Health upon request.
2. The registration fee per account will be waived if client agrees to contract for 12 (twelve) months of continuous active billing of the program per account. A $0 refundable deduction will be applied on the first payment, refundable at the 12th month term.
3. Client acknowledges monthly fees due per fee schedule as listed on xxx.0xxxxxx.xxxx website.
4. The client will pay Provider $7 for each office visit to each person on the account. Other procedures will occur an additional $7 charge per incidence, excluding allergy serum and some vaccines which are priced separately.
5. Billing of monthly fees will be made by Provider through automatic withdrawal from Client’s Checking, Visa, Mastercard, American Express or Discover account. Client understands that automatic withdrawal is the only way subscription fees can be paid. Checks or cash will not be accepted, Invoices will not be mailed. A discount applies if the membership is paid in yearly installments.
Client Fees. CONTRACTOR shall not charge a fee to DMC beneficiaries to whom services are provided pursuant to this Agreement, their estates and/or responsible relatives, unless a Share of Cost is determined per Medi-Cal eligibility.
