The following SCORING system will be used to determine, by “Evidence,” the contractor’s qualifications and competency: 0 = No or Insufficient Evidence: Zero (0) points are awarded to responses that either fail to provide the requested information or provide information which cannot be understood. Proposals that receive a score of “0” in any one section WILL NOT be approved regardless of the Contractor’s total average.

Samples shall be provided to the State at no charge. Failure to provide samples upon request, may result in rejection of the response.
