Draft Transition Plan. Based on the Self-Evaluation, the Consultant will prepare a draft Transition Plan for the Client. The plan will include:  Executive summary which will describe the project purpose, process, and most significant findings;  Summary and detailed findings of Self-Evaluation; and  A phased schedule with cost estimates for the removal of facility barriers that cannot be resolved through relocation of services, the provision of auxiliary aids, or equivalent facilitation. The Consultant will provide electronic copies of the Draft Transition Plan in Microsoft Word and Adobe PDF formats to the Client.

Draft Transition Plan. The Offeror shall provide a Draft Transition Plan that will be finalized after contract award. The Draft Transition Plan shall, at a minimum, include:

The plan will consist of: • Executive summary which will describe the project purpose, process, and most significant findings; • Documentation of Town efforts to-date related to ADA compliance; • Identification of the Town's ADA Coordinator; • Documentation of the Town's ADA Public Notice of Non-discrimination and Grievance Policy/ Procedure; • Summary of Town programs, services, activities, and facilities to be evaluated in future phases; • A budget and schedule for future phases; • Summary and detailed findings of Self-Evaluation; and • A phased schedule with cost estimates for the removal of facility barriers that cannot be resolved through relocation of services, the provision of auxiliary aids, or equivalent facilitation. The Consultant will provide electronic copies of the Draft Transition Plan in Microsoft Word and Adobe PDF formats to the Town.

  • Contract Transition Upon Contract expiration or termination, the Contractor shall ensure a seamless transfer of Contract responsibilities with any subsequent Contractor necessary to transition the Products and services of the Contract. The incumbent Contractor assumes all expenses related to the contract transition.

  • Implementation Plan The Authority shall cause to be prepared an Implementation Plan meeting the requirements of Public Utilities Code Section 366.2 and any applicable Public Utilities Commission regulations as soon after the Effective Date as reasonably practicable. The Implementation Plan shall not be filed with the Public Utilities Commission until it is approved by the Board in the manner provided by Section 4.9.
