Common use of Employee Requested Clause in Contracts

Employee Requested. Leave of absence for the duration of the disability for a period up to six (6) consecutive months shall be granted to any permanent employee who, as a result of an extended illness or injury including pregnancy or childbirth related disabilities, has exhausted his/her accumulation of sick leave. Upon the request of such employees, such leave may be extended up to a total maximum of one (1) year. In all cases a physician's statement indicating diagnosis, prognosis and estimated length of disability is required before such leave is granted. An employee returning from employee-requested medical leave shall be reinstated to his/her original job or to a position of like status, pay and seniority.
