Common use of Extended Health Benefit Clause in Contracts

Extended Health Benefit. Extended Health Benefits eligibility shall be the first of the month following six (6) months of employment. Each qualified employee shall be eligible for the Extended Health Benefit Plan at no cost to the employee. Each qualified employee will be issued a Health Benefits Swipe Card. Paramedicals to a combined maximum $1,000, with a deductible of $50, effective December 31, 2019. a) Adults - to a maximum of $300 per two (2) year period; and b) Children - to a maximum of $300 per one (1) year period.
