Eye Care Program. It is agreed that the STATE shall continue the Eye Care Program during the period of this Agreement. The Program shall be administered by the STATE and shall provide benefits to all eligible full-time unit employees and their eligible dependents (spouse and unmarried children under twenty-six (26) years of age who live with the employee in a regular parent-child relationship). The extension of benefits to eligible dependents shall be effective only after the employee has been continuously employed for a minimum of sixty (60) days. The Program shall provide for eligible full-time employees and eligible dependents as defined above to receive a $40 payment for prescription eyeglasses with regular lenses and a $45 payment for such glasses with bifocal lenses. Full-time employees and eligible dependents as defined above shall also be eligible for a maximum payment of $35 or the non-reimbursed cost, whichever is less, of an eye examination by an ophthalmologist or an optometrist. Proper affidavit and submission of receipts are required of the employee in order to receive payment. Each eligible employee and dependent may receive only one payment for glasses and one payment for examinations during the period from July 1, 2015 to June 30, 2017, and one payment for glasses and one payment for examination during the period from July 1, 2017 to June 30, 2019. The forms to be filled out by the employee for payment shall identify both the STATE and the Union, but shall be submitted directly to the College/University where the employee is employed.

2. Full-time employees and eligible dependents as defined above shall be eligible for a maximum payment of $35.00 or the non-reimbursed cost, whichever is less, of an eye examination by an Ophthalmologist or an Optometrist.

Each eligible employee and dependent may receive only one payment for examinations and one payment for glasses during the period from July 1, 2019 to June 30, 2021, and one payment for examinations and one payment for glasses during the period from July 1, 2021 to June 30, 202319. This program ends on June 30, 2023. Proper affidavit and submission of receipts are required of the employee in order to receive payment.
4. Upon submission of a certification and receipts, employees may obtain reimbursement under the Eye Care Program for the period of July 1, 2017 through June 30, 2021and for the period of July 1, 2021 through June 30, 2023.

The extension of benefits to dependents shall be effective only after the employee has been continuously employed for a minimum of sixty (60) days. Full-time employees and eligible dependants as defined above shall be eligible for a maximum payment of $35 on the cost, whichever is less, of an eye examination by an Ophthalmologist or an Optometrist. Each eligible employee and dependent may receive only one (1) payment for glasses and one payment for examinations during the period of July 1, 2011 to June 30, 2013, and one (1) payment for the period July 1, 2013 to June 30, 2015. This program ends on June 30, 2015. Proper affidavit and submission of receipts are required of the employee in order to receive payment.

2. Full-time Managers and eligible dependents as defined above shall be eligible for a maximum payment of $35.00 or the non-reimbursed cost, whichever is less, of an eye examination by an Ophthalmologist or an Optometrist.

Each eligible Manager and dependent may receive only one payment for examinations and one payment for glasses during the period from July 1, 2015, to June 30, 2017 and one payment for examinations and one payment for glasses during the period from July 1, 2017 to June 30, 2019. This program ends on June 30, 2019. Proper affidavit and submission of receipts are required of the Manager in order to receive payment.

This program ends on June 30, 2015. Proper affidavit and submission of receipts are required of the employee in order to receive payment.
Eye Care Program. No later than the first day of the month following hire, unless hire date is the first day of the month, then immediately, the Employer agrees to pay the full premium for optical insurance for all full-time and regular part-time employees and their dependents. Benefits shall consist of:
(a) One eye examination during a period of twenty-four (24) consecutive months for an adult covered by this plan. One eye examination during a period of twelve (12) consecutive months for children under 19 years of age. The benefits shall consist of the prevailing and customary charge less a $10.00 co-payment by the covered person.
(b) One pair of lenses or contact lenses during a period of twenty-four (24) consecutive months for an adult covered by this plan. One pair of lenses or contact lenses during a period of twelve (12) consecutive months for children under 19 years of age. The maximum benefit shall consist of $75 less a $10.00 co-payment by the covered person ($110.00 for contacts). Effective April 1, 1997, Bi-focals, tri-focals, etc., shall be increased by $25.00.
(c) Frames during a period of twenty-four (24) consecutive months for an adult covered by this plan. Children under 19 years of age, once during a period of twelve (12) consecutive months. The maximum benefit shall be $60.00.
(d) The Employer reserves the right to self-fund the eye care program provided the benefits remain the same.
