Failure Reporting. All non conformances and incidents are processed in accordance with the ARIANESPACE Quality Manual. Any incident during integration or test is registered in the log book of the equipment concerned. Assessment of incidents is performed systematically by reliability services of the contractors and by the Prime Contractor. In case of significant anomalies, visits to the main contractor’s facilities may be organized, if necessary. Significant incidents are also reported systematically during RAV and RAL reviews.
Failure Reporting. B▇▇▇▇▇ will maintain and relay to Bioheart quality data, inspection and test data, failure trends, and scrap data for each lot of Products manufactured. Each party shall promptly report to the other party in writing on failure trends of the Product that it identifies that might reveal problems in the Product. Bioheart shall be responsible for trend analysis and will inform B▇▇▇▇▇ of any investigation and/or corrective action necessary as a result of such analysis.
Failure Reporting. Whenever requested by Lam, ▇▇d with respect to specific identified failed Products, you will provide to Lam'▇ Commodity Management a worldwide Failure Analysis identifying the following:
10.1.1. Percentage of No-Trouble-Found ("NTF") of the failed Product over the past six months
10.1.2. Total number of assemblies of the failed Product repaired in the past six months
10.1.3. Component or assemblies you replaced in the failed Product that fixed the cause of the failure
10.1.4. Pareto of defective components used to repair the failed Product over the past six months
10.1.5. Engineering changes that have been released that would have affected the failure
10.1.6. Revision level of the Product assembly when received by Supplier
10.1.7. Latest revision level of the repairs to the failed Product
10.1.8. Date received by the Supplier
10.1.9. Date original Product was shipped to Lam
