Significant Incidents. In addition to notifying the appropriate authorities, Grantee will submit notice to the SUD email box, XxxxxxxxxXxxxx.Xxxxxxxxx@xxxx.xxxxx.xx.xx and Substance Use Xxxxxxxx@xxxx.xxxxx.xx.xx significant incidents involving substantial disruption of Grantee’s program operation or affecting or potentially affecting the health, safety or welfare of the System Agency funded clients or participants within three (3) calendar days of discovery.
Significant Incidents. In addition to notifying the appropriate authorities, Xxxxxxx will notify the assigned contract manager to this Contract in writing any significant incidents involving substantial disruption of Grantee’s program operation or affecting or potentially affecting the health, safety or welfare of the System Agency funded clients or participants within three (3) calendar days of discovery.

Significant incidents may include, but are not limited to: client death; overdose; assaults; or any other related harm to clients in services; and any reason that leads to a facility closure or disruption in services, such as those identified in Additional Provisions, Section 6, Disaster Services.
Significant Incidents. In addition to notifying the appropriate authorities, Contractor shall report to the contract manager assigned to the Contract significant incidents involving substantial disruption of Contractor’s program operation or affecting or potentially affecting the health, safety or welfare of Department-funded clients or participants within 72 hours of discovery.
Significant Incidents. Performing Agency will report any incidents that result in substantial disruption of program operation involving or potentially affecting persons served to the System Agency Contract Manager within seventy-two (72) hours.

In addition to notifying the appropriate authorities, Contractor shall report to County significant incidents involving substantial disruption of Contractor’s program operation or affecting or potentially affecting the health, safety, or welfare of County funded clients or participants within seventy-two (72) hours of discovery.

Significant Incidents. (a) If:
(i) a significant incident occurs within the facility; and
(ii) the incident had or has the potential to prevent or restrict Ausgrid’s ability to meet its obligations under this agreement or the NER, then the parties must meet as soon as possible after the occurrence of the incident to discuss and resolve any relevant issues.
(b) On or prior to the meeting convened under clause 7.4(a) (Incident meeting), the parties must provide each other with all relevant information within their possession in relation to the incident.
(c) The parties will prepare and exchange a written report (Incident report) which contains the following:
(i) a description of the incident and its impact;
(ii) the cause of the incident and any relevant findings; and
(iii) recommendations to mitigate or prevent a recurrence.
(d) If the relevant incident has or has the potential to prevent or restrict Ausgrid's ability to meet its obligations under the NER, the parties must meet and negotiate in good faith to agree any necessary or appropriate amendments to the terms and conditions of this agreement, to ensure compliance.
