No concession items except charitable efforts. Give-away samples are allowed. Plastic gloves are recommended. The Radisson facilities must not be used for any storage or preparation of food items at the NH Farm & Forest Expo. If you are planning to give away food samples, or sell packaged food items you must have paperwork on file with the Manchester Health Department. Manchester Health Department paperwork will be sent to you in early January 2017. Please note: All food exhibitors must display or sample NH-made food products, or products are made mostly from NH ingredients.

A copy of your Department of Health certification and a copy of your most recent facility health inspection form (required by the Manchester Department of Health). Please be aware the Manchester Department of Health will be onsite during the Expo. If your certification and health inspection form is not received by the Manchester Department of Health by Expo Management by Friday, January 19th, 2018 your organization’s ability to exhibit at the 2018 NH Farm & Forest Expo will be revoked with no refund of booth fees. Insurance Certification Information We require that a Certificate of Insurance be provided. The policy must be a $1 Million Dollar General Liability policy issued by a company authorized to do business in New Hampshire. This is a simple (one page) document which is issued as a matter of information only from your general liability insurance policy. You should be able to obtain one at no extra cost to you from your insurance company. This Certificate of Insurance must contain the NH Farm & Forest Expo dates of February 1 – 3, 2018 and list the NH Farm & Forest Expo as Additional Insured. This form must be received by Friday, January 26th, 2018. If not received, your organization will not be allowed access to the Expo floor. If your organization is self-insured, please provide a statement on your organization’s letterhead stating your insurance status, signed by an official at your organization. Insurance certifications or statements of self-insurance may be faxed to (000)000-0000 or e-mailed to xxxx@xxxxxxxxxxxxxxxxxxx.xxx.

  • ORDERING REQUIREMENTS Eligible Purchasers shall order Goods and/or Services from this Contract, consistent with the terms hereof and by using any ordering mechanism agreeable both to Contractor and Purchaser but including, at a minimum, a purchase order. When practicable, Contractor and Purchaser also shall use telephone orders, email orders, web- based orders, and similar procurement methods (collectively “Purchaser Order”). All Purchase Orders must reference the Contract number. The terms of this Contract shall apply to any Purchase Order and, in the event of any conflict, the terms of this Contract shall prevail. Notwithstanding any provision to the contrary, in no event shall any ‘click-agreement,’ software or web-based application terms and conditions, or any other agreement modify the terms and conditions of this Contract.

  • Filing Requirements Escrow securities will not be released under this Part until the Issuer does the following: (a) at least 20 days before the date of the first release of escrow securities under the new release schedule, files with the securities regulators in the jurisdictions in which it is a reporting issuer (i) a certificate signed by a director or officer of the Issuer authorized to sign stating (A) that the Issuer has become an established issuer by satisfying one of the conditions in section 3.1 and specifying the condition, and (B) the number of escrow securities to be released on the first release date under the new release schedule, and (ii) a copy of a letter or other evidence from the exchange or quotation service confirming that the Issuer has satisfied the condition to become an established issuer; and (b) at least 10 days before the date of the first release of escrow securities under the new release schedule, issues and files with the securities regulators in the jurisdictions in which it is a reporting issuer a news release disclosing details of the first release of the escrow securities and the change in the release schedule, and sends a copy of such filing to the Escrow Agent.

Practitioner shall screen all current Covered Persons against the Exclusion List within 30 days after the Effective Date and on a monthly basis thereafter.‌ c. Practitioner shall require all Covered Persons to disclose immediately if they become an Ineligible Person.‌ Practitioner shall maintain documentation in order to demonstrate that Practitioner: (1) has checked the Exclusion List (i.e., a print screen of the search results) and determined that its Covered Persons are not Ineligible Persons; and (2) has required its Covered Persons to disclose if they are an Ineligible Person. Nothing in this Section III.D affects Practitioner’s responsibility to refrain from (and liability for) billing Federal health care programs for items or services furnished, ordered, or prescribed by an excluded person. Practitioner understands that items or services furnished by excluded persons are not payable by Federal health care programs and that Practitioner may be liable for overpayments and/or criminal, civil, and administrative sanctions for employing or contracting with an excluded person regardless of whether Practitioner meets the requirements of Section III.D.

  • Child Abuse Reporting Requirements A. Grantees shall comply with child abuse and neglect reporting requirements in Texas Family Code Chapter 261. This section is in addition to and does not supersede any other legal obligation of the Grantee to report child abuse. B. Grantee shall use the Texas Abuse Hotline Website located at xxxxx://xxx.xxxxxxxxxxxxxx.xxx/Login/Default.aspx as required by the System Agency. Grantee shall retain reporting documentation on site and make it available for inspection by the System Agency.
