Formal Application. In order to participate in the plan, an eligible employee must make written application to the Board on or before March 31 of the current school year, stating the date when the eligible employee wishes the monthly deferrals (deductions) to commence.
Formal Application. In order to participate in the plan, an eligible employee must make written application (Form A) to the Superintendent by May 31st of the year in which the deferrals are to commence.

In order to participate in the Plan, an Eligible Teacher must make written application by way of Schedule "B" to the Superintendent of schools on or before March 31, or at a date otherwise agreed between the School Division and the Participant, stating the date of participation in the Plan and the school year in which the Leave of Absence is to be taken.
Formal Application. To proceed to the formal application phase, the applicant is required to submit to the CAA the documents listed in Section C, Appendix 2, Table C-2 under the heading titled Initial Application Documents, Formal Application.
2.3.1 The FAA shall inform the CAA as to what the appropriate FAA ratings are and what the certificate number will be.
a. The CAA shall review the formal application to ensure completeness and acceptability. This will include an initial review of the proposed FAA Supplement in comparison with the sample in Section C, Appendix 1.
b. The FAA Supplement, along with the MOE, sets out the structure and procedures of the repair station to meet the requirements of 14 CFR part 145. The FAA Supplement (as seen in the example in Section C, Appendix 1) must be customized to the repair station procedures and accurately describe how the repair station meets the FAA Special Conditions.
c. Instructions and a sample of the FAA Supplement are contained in Section C, Appendix 1.
Formal Application. 2.1. In order to participate in the Plan, an Eligible Teacher must make written application by way of Schedule "B" to the superintendent of schools on or before March 31st, or at a date otherwise agreed between the Employer and the Association, stating the date when the Eligible Teacher wishes to participate in the Plan and the school year in which the Leave of Absence is to be taken.
2.2. The approval of each application made under clause 2.1 shall rest solely with the Employer. The superintendent of schools shall, by May 15th of that year, or at a date otherwise agreed between the Employer and the Association, advise each applicant of the Employer 's approval or disapproval of their application and if the latter, an explanation, therefore.
2.3. If the Employer gives its approval in accordance with clause 2.2, the participation of the Eligible Teacher in the Plan will become effective on the date requested by the Eligible Teacher, or if such date is not agreed to by the Employer, then on a date which is agreed to by the Employer and the Eligible Teacher.
Formal Application. Based upon the approved Pencil Draws, the Payment Applicant shall prepare its formal Payment Applications, which shall be supported by such data substantiating its right to payment as Owner may require, such as copies of requisitions from Subcontractors and Suppliers. Such applications may not include requests for payment of amounts the Payment Applicant does not intend to pay to its Subcontractors or Suppliers because of a dispute or other reason, unless the Payment Applicant has specifically notified Owner of an intended backcharge or dispute, and Owner has nonetheless approved the payment.
Formal Application. Performance audition in student’s principal area of performance (vocal or instrumental) to include appropriate solo repertoire, scales, studies, sight reading;
Formal Application. In order to participate in the plan, an eligible employee must make written application by way of schedule "A" to the Committee at least three months prior to the requested commencement of deferrals under the plan or at a date otherwise agreed between the Board and the Local, stating the date when the eligible employee wishes the deferrals to commence.
Formal Application. 1. Formal application must be reviewed within 30 days from the date of issuance of the Initial Determination Letter by the County.
2. The Applicant has the right to submit an application after the Initial Determination regardless of the conclusion of the Initial Determination.
3. The Development Agreement Application form is available from the Planning Division or online at xxx.xx.xx-xxxxx.fl.us. The following information is required to be submitted with a completed Development Agreement Application (submit the original and 10 copies of the application and all exhibits):
a. Proposed Development Agreement meeting all requirements of the St. Xxxxx County Land Development Code Part 5.05.00 and Chapter 163.3227 Florida Statutes.
b. Legal Description (attach as Exhibit A to the Development Agreement).
c. Proof of ownership (copy of deed or purchase agreement, and land title report); land title report shall be considered valid for 3 months and will need to be updated thereafter.
d. General location map with subject property clearly identified.
e. Traffic Analysis (from Concurrency Application), as applicable; Traffic Analysis will need to be updated if data used is updated.
f. Conceptual design for all proposed infrastructure improvements (attached as Exhibit B to the Development Agreement).
g. Cost estimates for all proposed infrastructure improvements, if impact fee credits are being sought as apart of the Agreement (attached as Exhibit C to the Development Agreement).
