The Bank Secrecy Act, a federal law, requires all financial institutions to report large currency transactions to the IRS. The Bank will prepare a Currency Transaction Report (CTR) at the time the transaction occurs and forward it to the IRS. In order to complete the report, we are required to obtain certain information about the individual presenting the transaction as well as the individual for whom the transaction is being conducted.

1. stop Payment order request: You may request a stop payment order on any check drawn on your Account. To be binding a stop payment order must be dated, signed and describe the Account check number and exact amount. If the stop payment order is placed by telephone a recording of the call will be taken and verification sent to address of record. The stop payment will be effective if the Credit Union receives the order in time for the Credit Union to act upon the order and you accurately and timely describe the Account number, date of the check, payee name, number of the check and its exact amount. You understand that the exact information is necessary for the Credit Union’s computer to identify the check. If you give us incorrect or incomplete information, we will not be responsible for failing to stop payment on the check. We must be given reason- able opportunity to act on any stop payment order before it can be considered effective. If the stop payment order is not received in time for us to act upon the order, we will not be liable to you or to any other party for payment of the check. If we re-credit your Account after paying a check over a valid and timely stop payment order, you agree to sign a statement describing the dispute with the payee, to transfer to us all of your rights against the payee or other holders of the check and to assist us in any legal action. We are not obligated to reimburse you for a paid stop payment unless you can provide proof of loss associated with such payment.
2. duration of order: You may make an oral stop payment order, which will lapse within fourteen (14) calendar days unless confirmed in writing within that time. A written stop payment order is effective for six (6) months unless renewed in writing and that written notice is given to the Credit Union. We do not have to notify you when a stop payment order expires.

We must receive the notice in time for us to have a reasonable opportunity to act on it, and you must tell us the precise date of the check, amount, check number and payee. We are not responsible for any unauthorized signature or alteration that would not be identified by a reasonable inspection of the item. If you purchase your blank checks from a third party other than us, you should first consult with us to ensure that we will accept the checks you plan to purchase. We may deny any form of checks that we determine in our discretion would cause additional risk to us or that we would not be able to process efficiently. We may refuse any withdrawal or transfer request on forms of checks not approved by us or by a method we do not permit. If a person who is not our deposit or loan customer tries to cash your check at any of our branches, we may charge them a fee or refuse to cash it. We also may require that they provide us identification we deem acceptable. We may pay the amount encoded on your foreign check in U.S. dollars, even if you wrote the check in a foreign currency or made a notation on the check’s face to pay it in a foreign currency. You agree that in the event you request a large cash withdrawal, as determined by us in our sole and absolute discretion, you are solely responsible for your security once you take possession of your cash withdrawal. You additionally agree to execute, at our request, a release and indemnity agreement. In addition to the foregoing, if any person should instruct the Credit Union to make a large cash withdrawal (the definition of which shall be within the Credit Union’s sole discretion) in lieu of other methods of transferring funds offered by the Credit Union, the Member hereby releases the Credit Union from any and all liability, claims and demands whatsoever which arise from or in any way relate to the Member’s possession of any large sums of cash from the moment possession of same is made available to the Member; and Member will execute and deliver any further releases the Credit Union may require in connection with any such withdrawal.
