Local Service Request (LSR). The industry standard form, which contains data elements and usage rules, used by the Parties to establish, add, change or disconnect resold services and unbundled elements for the purposes of competitive local services.
Local Service Request (LSR). The Ordering and Billing Forum document designated by CenturyLink to be used by the Parties to establish, add, change or disconnect local Telecommunications Services for the purpose of providing competitive local Telecommunications Services. Sometimes referred to as a Service Order.
Local Service Request (LSR). Positive acknowledgement of receipt of a non-valid LSR will be made within two business days, provided the LSR is received before 3PM Eastern Standard Time (1PM Mountain Standard Time). The start time for determining the Local Service Request Confirmation (LSCN) interval will commence with receipt of a valid LSR. A non-valid LSR will not start the LSCN interval.

Unless otherwise agreed by the Parties, the Local Service Request document will be used by the Parties to initiate carrier-to-carrier arrangements related to each Party’s number portability obligations. The document may also be used for the exchange of subscriber information, in conjunction with each Party’s obligations under this Agreement.
Local Service Request (LSR). LSRs that are subject to the FCC’s number portability interval requirements shall be processed in accordance with those requirements. Except for LSRs that are subject to the FCC’s number portability interval requirements, positive acknowledgement of receipt of a non-valid LSR for a Non-Simple Port request will be made within 24 hours, provided the LSR is received before 3PM Eastern Standard Time. Non- Simple Port LSRs received after the 3PM cutoff will be considered to be received on the following business day. Except for LSRs that are subject to the FCC’s number portability interval requirements, positive acknowledgement of receipt of a non-valid LSR for a Simple port request will be made within 4 hours, provided the LSR is received by 1P.M. local time in the predominant Time Zone of the NPAC Region where the End User’s telephone number is being ported. Simple Port LSRs received after the 1PM cutoff will be considered to be received on the following business day, and the response clock starts at 8 A.M. (local time in the predominant time zone of the NPAC Region where the number is being ported).
Local Service Request (LSR). The documents used by the Parties to establish, add, change or disconnect local Telecommunications Services for the purpose of providing competitive local Telecommunications Services.
Local Service Request (LSR). 1.2.1 The NSP shall submit a Local Service Request (LSR) to the OSP according to standard practices as set forth in Section 8 of the General Terms and Conditions, for purposes of requesting Local Number Portability (LNP). The required fields of the LSR consist of: Account Number, Telephone Number, Zip Code, Customer Carrier Name Abbreviation, Purchase Order Number, Desired Due Date/Time, Requisition Type, Company Code, Telephone Number (Initiator), New Network Service Provider, End User Customer name, address and Supplemental Order Notice and Version (if applicable). The OSP may charge an LSR charge in accordance with the applicable UBET Tariff that lists a specific charge applying to LSRs for Local Number Portability. In the event that UBET proposes to change one of its Utah State Tariffs to implement a specific LSR charge, UBET shall provide Bresnan advanced notice of any such proposed Tariff change or such LSR charges shall not be charged to either party during the term of this Agreement.
1.2.2 The Parties agree that the standard interval for an LNP Simple Port request shall not exceed 4 business days from the OSP’s receipt of a complete and correct LSR, or the interval applicable to wireline carriers.
Local Service Request (LSR). 12.2.1.1.1 Qwest shall provide electronic interface gateways for submission of LSRs, including both an application to application interface and a Graphical User Interface (GUI).
12.2.1.1.2 The interface guidelines for the application to application interface are based upon the Order & Billing Forum (OBF) Local Service Order Guidelines (LSOG), and the appropriate electronic transmission standards. Exceptions to the above guidelines/standards shall be specified in the Interconnect Mediated Access (IMA) disclosure documents.
12.2.1.1.3 The GUI shall provide a single interface for Pre-Order and Order transactions from WSP to Qwest and is browser based. The GUI interface shall be based on the LSOG and utilizes a WEB standard technology, Hyper Text Markup Language (HTML), JAVA and the Transmission Control Protocol/Internet Protocol (TCP/IP) to transmit messages.
12.2.1.1.4 Functions Pre-Ordering - Qwest will provide real time, electronic access to pre-order functions to support WSP’s ordering via the electronic interfaces described herein.
Local Service Request (LSR). Orders for resale of services will be placed utilizing standard LSR forms as per current Local Service Ordering Guide format. Complete and accurate forms must be provided by Alltel before a request can be processed.
3.2.1. Golden West will accept orders for As-Is Transfer (AIT) of services from Golden West to Alltel where Golden West is the end-user’s current local exchange company.
3.2.2. Alltel will be the customer of record for all Services purchased from Golden West. Except as specified herein, Golden West will take orders from, xxxx and expect payment from Alltel for all Services ordered.
