Mandated Reporter. The Network understands that all of its employees, board members, agents and contractors are mandatory reporters under Colorado state law, and that the Network shall be responsible for ensuring that the appropriate training is delivered.
Mandated Reporter. I understand that as a Licensed Mental Health Counselor, Xx. Xxxxxxxx is considered by state law to be a mandated reporter. She is therefore obligated to report to the proper authorities any evidence of physical or sexual abuse or neglect of minors, elders, or vulnerable adults; or any direct threat to harm oneself or another person.
Mandated Reporter. All contract employees performing work related to this agreement are considered to be mandated reporters with reference to suspicions of child maltreatment; and, are therefore required to report suspicions of child abuse or neglect to Marathon County Department of Social Services or a local law enforcement agency.
Mandated Reporter refers to a person who has regular contact with a vulnerable person and observes, suspects and/ or knows of child abuse, neglect or exploitation. A Mandated reporter is required by O.C.G.A. 19-7-5 to report under penalty of the law for failure to report.
Mandated Reporter. The CONTRACTOR shall complete the mandated reporting training program provided by the District before serving studentsfrom the effective date of this contract.
Mandated Reporter. As a child care provider, I am required to report any suspected child abuse or neglect.
Mandated Reporter. An individual required to report observed or suspected child abuse or neglect to designated law enforcement or social services agencies.

Mandated Reporter. I agree to monitor and promote the welfare of clients seen by supervisee and promote the development of the supervisee’s professional competence. I am aware that this is the policy of DCF that: Each employee/consultant must receive at a minimum, 15 minutes of individual face‐to‐face clinical supervision per month per TSS/SS case assigned; and at a minimum 15 minutes per month individual or group face‐to‐face clinical supervision per case assigned for Supervised Visitation (SV) cases. Supervision shall be provided by a licensed behavioral health practitioner in good standing for employees/consultants who provide Supervised Visitation services. Supervision shall be provided by a Master’s level clinician or licensed clinician for employees/consultants who provide TSS/SS services. Mandated Reporter. All Board employees will receive Mandated Reporter training as required by law. Training shall occur on scheduled institute days during contract time. All Employees are considered mandated reporters and shall act according to that law.
