Media Release Form. The City of Sunny Isles Beach (the “City”) and its authorized employees or agents may be taking photographs of Participants in Activities. This documentation may be used in future City sponsored brochures, posters, pamphlets, newspaper, photographs, City advertisements and/or any other promotional materials. To ensure your privacy, we would like your permission to include you in these promotional materials. I, (“NAME”) agree to give permission for photographs to be used in future promotional materials by the City. I agree that any photographs, pictures, slides, movies, or videos may be taken in connection with my participation in the event or activity without any compensation from the City or their agents and employees and I do hereby consent to the use of said photographs, pictures, slides, movies, or videos for any legal purpose. In order to ensure a safe and fun experience for everyone, the following rules will be enforced. • Any acts of violence by a participant or their parent/guardian will not be tolerated and may be grounds for immediate removal from the program. • Use of bad language such as curse words or threats by participants or their parent/guardian is not permitted. • Fighting of any kind by participants or their parent/guardian, whether verbal or physical, will not be tolerated or allowed and may be grounds for immediate dismissal. Continued offenses may be grounds for removal from the program. • Lewd (obscene, dirty, off color, suggestive) behavior is not permitted. • Destruction or defacement of property is not permitted. • Illegal use or possession of drugs or alcohol is not permitted. • Smoking during program activities is not permitted. • Misbehavior of any kind during group transportation, while in a program, which causes safety concerns is not permitted. • Firearms, weapons, or destructive devices of any kind are not permitted. The Cultural & Community Services Department employees and their designees have authority to regulate participants’ behavior and may have individuals sent home for a period of time up to and including dismissal from the program if they do not comply with standard conduct rules. Media Release Form. The intention of this document is to advise you of the use of still and video photography during BIOS education programs, and to obtain your consent to use your name, likeness, and testimonial(s) as a part of our educational outreach efforts. All media resources (still images, video, and audio) will remain the property of BIOS and will not be used by BIOS or its representatives for any commercial value or to receive monetary gain. Finished video clips and still images may be made available on the BIOS website and social media accounts, as well as incorporated into presentations and both digital and printed promotional materials. System Agency does not endorse any vendor, commodity, or service. Grantee is not authorized to make or participate in any media releases or public announcements pertaining to this Grant Agreement or the Services to which they relate without System Agency’s prior written consent, and then only in accordance with explicit written instruction from System Agency.

  • PHOTOGRAPH RELEASE You give your permission to us to use any photograph or photographic image including video or video stills taken of you while you are in any Common Areas of the Community or at any Community sponsored events.   • Photo Release The Resident grants to University permission to: record Resident's participation and appearance on videotape, audiotape, film, photograph or any other medium; use Resident's name, likeness, voice and biographical material in connection with such recordings; exhibit or distribute such recordings in whole or in part without restrictions or limitation for any legal purpose, including without limitation educational or promotional purposes, which the University and those acting pursuant to its authority deem appropriate; and copyright such recordings in its own name or to publish, to market and to assign without consideration, compensation or report to Resident.

  • Photographic Release Resident gives permission to Owner and Management, to use, without liability or remuneration, any photograph or photographic image taken of the Resident while participating in Building sponsored events, or while the Resident is in the common areas, public spaces, grounds, or offices of the Building. I understand that Growing Kids Learning Center will not associate the photographs or video or audio recordings with my child’s full name unless I first give written consent. Authorized Pick Up - Growing Kids Learning Center will only release my child to me or to a person that I name, using the check in and check out process provided by the center. Both I and any person whom I have named are responsible to inform a member of the Growing Kids Learning Center staff of my child’s arrival and departure. I will not share access codes, computer passwords, or other security measures with anyone unless I have written authorization from Growing Kids Learning Center in advance.

  • Waiver and Release In consideration for the granting of the Restricted Stock Units, the Employee hereby waives any and all claims whether known or unknown that the Employee may have against the Company and its Subsidiaries and Affiliates and their respective directors, officers, shareholders, agents or employees arising out of, in connection with or related to the Employee’s employment, except for (1) claims under this Agreement, (2) claims that arise after the date hereof and obligations that by their terms are to be performed after the date hereof, (3) claims for compensation or benefits under any compensation or benefit plan or arrangement of the Company and its Subsidiaries and Affiliates, (4) claims for indemnification respecting acts or omissions in connection with the Employee’s service as a director, officer or employee of the Company or any of its Subsidiaries and Affiliates, (5) claims for insurance coverage under directors’ and officers’ liability insurance policies maintained by the Company or any of its Subsidiaries or Affiliates, or (6) any right the Employee may have to obtain contribution in the event of the entry of judgment against the Company as a result of any act or failure to act for which both the Employee and the Company or any of its Subsidiaries or Affiliates are jointly responsible. The Employee waives any and all rights under the laws of any state (expressly including but not limited to Section 1542 of the California Civil Code), which is substantially similar in wording or effect as follows: A general release does not extend to claims which the creditor does not know or suspect to exist in his favor at the time of executing the Release, which if known by him must have materially affected his settlement with the debtor. This waiver specifically includes all claims under the Age Discrimination in Employment Act of 1967, as amended. The Employee acknowledges that the Employee (a) has been advised to consult an attorney in connection with entering into this Agreement; (b) has 21 days to consider this waiver and release; and (c) may revoke this waiver and release within seven days of execution upon written notice to Legal Counsel, Employment and Labor, Law Department, Unum Group, 0 Xxxxxxxx Xxxxxx, Xxxxxxxxxxx, Xxxxxxxxx 00000. The waiver and release will not become enforceable until the expiration of the seven-day period. If the waiver and release is revoked during such seven-day period, the grant shall be void and of no further effect.
