Medical Assistance Services. Medical Referrals If a medical emergency arises while travelling, you can contact our Administrator Emergency Assistance Centre and you will be referred to the nearest designated physician or medical facility. Medical Consultation and Monitoring Our Administrator’s network of medical professionals is available 24 hours a day, 365 days a year, to consult with your attending physician to ensure that your medical needs are being met. Our Administrator’s network of medical professionals is experienced in working with physicians outside of Canada to determine the adequacy of care being received and the need for further assistance. Medical Transportation When our Administrator, in consultation with its network of medical professionals and in conjunction with your attending physician, determine that transfer to another medical facility is necessary, our Administrator will coordinate all aspects of the transport to and from the hospital and airport, at the point of departure and arrival. Our Administrator Assistance Coordinators will arrange for qualified medical accompaniment, if necessary. Neither The Toronto-Dominion Bank, TD Life Insurance Company or any other insurer, nor our Administrator is responsible for the availability, quality or results of any medical treatment you receive or fail to receive for any reason.

Medical Assistance Services. As specified in 18 NYCRR Sections 494.5(b) and 505.35(h)(1), Medical Assistance services include personal care services; home health aide services; personal emergency response services; nursing services; physical therapy; occupational therapy; speech therapy; medical supplies and equipment not requiring prior approval; and adult day health care provided in a program approved by the Commissioner of Health. The Provider is responsible for assuring that Medical Assistance services provided in the Assisted Living Program are furnished only to those Medical Assistance recipients who meet the eligibility requirements for the Assisted Living Program, as specified in 18 NYCRR Section 494.4 and with respect to whom the Social Services District has prior authorized payment for Medical Assistance services. The Provider will comply with such other provisions of 18 NYCRR Sections 494.4(h) and 505.35 as well as any other provisions of the Department's or the Department of Health's regulations as are relevant to the provision of services to Medical Assistance recipients participating in the Assisted Living Program.

Medical Assistance Services provides emergency response in any major language • referrals to an appropriate physician, clinic or hospital • confirms you have coverage • guarantees or arranges payment to the hospital or physician for eligible expenses • provides assistance in contacting your family, place of business or family physician • supervises the medical treatment and keeps the family informed • arranges for transportation of a family member to the patient’s bedside • arranges for transportation home of the patient
Medical Assistance Services. We or Our Assistance Service Provider will provide assistance or advice to the Insured person, of the description specified below, in relation to a medical emergency: • Medical Practitioner/Hospital Referral We or Our Assistance Service Provider will provide, upon request, with the name, address, telephone number and, if available, office hours of Medical Practitioners, Hospitals, or any clinics, dentists and dental clinics (collectively "Medical Service Providers"). The final selection of any Medical Practitioner, Hospital, or Medical Service Provider shall be at the discretion of the Insured Person. While We or Our Assistance Service Provider shall exercise care and diligence in making any referrals, We or Our Assistance Service Provider shall not be responsible for any medical diagnosis or treatment provided by such Medical Practitioners, Hospitals, or Medical Service Providers, and cannot guarantee their quality.
