Monthly Service Fees. Fees for service ordered by You shall begin on the date of the initial order and that date shall serve as the monthly anniversary date (“Anniversary Billing Date”) for all future xxxxxxxx including one time fees, upgrades, additional services, cancellations and service credits. Fees are due in advance of the monthly service cycle and will be billed on the anniversary date of each month.
Monthly Service Fees. Customer agrees to pay the monthly service fees, as set forth in the Service Order, effective once Agreement is signed and upon Completed Installation of Service. At the time of Completed Installation, Customer will be billed the installation fee (if applicable), equipment costs (if applicable), the prorated monthly service charge, as well as the first full month of Service. Thereafter, the Service fee will be billed to the Customer monthly, in advance. All payments shall be due in full and within 20 days after receipt of invoice.
Monthly Service Fees. The Member must pay the following monthly service fee based upon his/her age:
4.1.1 Children under the age of 18. If the Member is under the age of 18 the Member’s monthly service fee is $57.00 (before discounts).
4.1.2 Between the age of 19-50. If the Member is between the ages of 19-50 the Member’s monthly service fee is $77.00 (before discounts).
Monthly Service Fees. 19.3.1 The Monthly Service Fee (Subscription) is a monthly fee, which includes a number of predefined Services - as further described in Schedule DD (Services and Deliverables). The monthly service fees are – unless specifically set out in Schedule DD (Services and Deliverables) – not affected by the actual consumption.
Monthly Service Fees. In consideration of Castlight’s provision of the Castlight Services (including the Castlight Services to *** Users) under Section 2 of this First Addendum, for each month (or portion thereof) during the Term (as defined in the MSA) after the Launch Date, Customer will pay Castlight, in accordance with Section 11, the Service Fee (as calculated under this Section 10).
Monthly Service Fees. The “Service Fee” for each month commencing with the Launch Date will be the sum of:
i. the product of: (A) the number of eligible *** Employee Users each month; and (B) the per *** Employee User per month rate of $*** (the “Monthly *** Employee Fee”); plus
ii. the product of: (A) the number of eligible *** Adult Dependent Users each month; and (B) the per *** Adult Dependent User rate of $*** (the “Monthly *** Dependent Fee”); plus
iii. the product of: (A) the number of eligible *** Users each month (which is the sum of the *** Employee Users and the *** Adult Dependent Users); and (B) the per *** User per month rate of $*** (the “Monthly *** Fee”); plus
iv. the product of: (A) the number of eligible *** Users each month; and (B) the per *** User per month rate of $*** (the “Monthly *** Fee”); provided that following the *** Launch Date, the Monthly *** Fee during the remainder of the Initial Term shall be the product of (x) the number of eligible *** Users each month and (y) $***, commencing with the first day of the first month following the *** Launch Date.
Monthly Service Fees. Provider’s monthly prices per end-user are made available off the website and its following links at xxx.xxxxxx.xxx . Provider reserves the right to change or modify these prices if our costs increase. Should Provider exercise its right to raise prices, the following conditions will apply: Provider must show in writing that its costs have increased to each Customer that requests this information and Provider may only raise prices the same percentage as Providers costs have increased. Should the Customer not contest the price increase within 30 days the customer agrees to be bound by the terms of this contract at the new rate. If the Customer contests the price increase and Provider can show in writing that its costs have increased, Customer agrees to be bound by the term of the agreement at the new costs. If Provider cannot prove in writing that its costs have increased, Provider shall continue providing customer the service at the old rate.
Monthly Service Fees. Subscriber agrees to pay a monthly service fee (“Monthly Service Fee”) as provided on the LTE 4G Cellular Internet Service Activation Form and Agreement, which is incorporated herein as if stated in full. The Monthly Service Fee shall include an initial 1GB of data per billing cycle. All Monthly Service Fees subsequent to the first month shall be invoiced to the Subscriber only a monthly cycle.
Monthly Service Fees. The Member must pay the following monthly service fee based upon his/her age:
4.1.1 Children under the age of 18. If the Member is under the age of 18 the Member’s monthly service fee is $69.00.
4.1.2 Above the age of 19+, the Member’s monthly service fee is $99.00.
4.1.3 Family Plan Members assume responsibility for all financial obligations of family members enrolled with them `in this agreement
4.1.3.1 Couples plan: defined as two adults of any change, no children $ 150
4.1.3.2 Family plans : defined as two adults and two children (under age of 18) $ 299
4.1.3.2.1 Two Parent Family Plan defined as up to 2 adults of any age plus one child $250
4.1.3.2.2 Single Parent Family Plan defined as up to 1 adult of any age plus one child $130
4.1.3.2.3 Additional children under the age of 18 may be added to any Family plan for $40 for each additional child.
