Multi-Disciplinary Team (MDT. Experts representing a range of providers who offer collaborative services through a single source. In this instance, the law requires that a county opting into the CSEC Program form an MDT to coordinate the case planning, case management, and services for each CSE youth. Promising practices suggest that an MDT be formed and individualized for each youth that is strengths-based and prioritizes the youth’s voice in the decision-making process. Youth should have the option of participating in the MDT as appropriate. Other public agency partners may be required to participate. Some of the activities the MDT may utilize are:  Convening emergency meetings upon identifying a CSE youth  Addressing the child’s basic needs  Making a recommendation about where to place the child  Developing a safety plan for the child  Reconvening should a triggering event occur (such as running away or another arrest).  Providing a venue for the legal exchange of confidential information (see Appendix A).

  • Disciplinary Investigations An employee who is the subject of a disciplinary investigation shall be informed in writing when the investigation is complete and of the determination of the investigation.

  • Disciplinary Hearing In the event the Union disagrees with the disciplinary action, the Union may appeal the matter to the Town Supervisor. The appeal must be submitted, in writing, within seven calendar days from receiving the Notice of Discipline.   • Discipline Procedure To ensure that there is no misunderstandings when discipline is to be administered the Parties shall adhere to the following procedure in each instance of discipline.

  • Pre-Disciplinary Meeting The College will schedule a pre-disciplinary meeting to permit the faculty member to respond to a notice of intent to discipline. At the beginning of any pre-disciplinary meeting, the College will describe its proposed discipline and the general reasons for issuing the proposed discipline.

  • Disciplinary Meetings In the event a meeting is held for disciplinary purposes, the affected employee shall have the right to have a Union xxxxxxx and/or Union representative present.
