Notice to District. Employees who receive a summons for jury duty are to notify Human Resources immediately of the proposed dates of service.
Notice to District. Using the District’s Certification of Health Care Provider form for pregnancy disability leave, a unit member should provide at least 30 days advance notice or notice as soon as practicable of the unit member’s need for pregnancy disability leave or need for reasonable accommodation based on the advice of her health care provider that reasonable accommodation is medically advisable because of pregnancy or a related medical condition.
Notice to District. 1. Employees absent due to a work-related disability shall keep the District informed of their condition, treatment, improvement, and expected date of return to duty.
2. When employees are physically able to resume normal duties, they shall be required to provide the District with their treating physician’s verification, subject to confirmation by a District paid medical examination.
Notice to District. 1.6.1. The employee must provide written notice to the District as far in advance of the leave as possible and as soon as the employee reasonably knows of the need for the leave. If the need for the leave is foreseeable based on an expected birth, placement for adoption or xxxxxx care or planned medical treatment, the notice must be provided at least thirty (30) calendar days in advance of the leave, or if not reasonably known thirty (30) calendar days before the leave, then as soon as reasonably practicable.
1.6.2. If an employee fails to give 30 days notice for foreseeable leave with no reasonable excuse for the delay, the District may deny the family care and medical leave request until at least 30 days after the date the employee provides notice to the District of the need for family care and medical leave. (29 CFR §825.304(b))
1.6.3. The employees shall consult with the District and make a reasonable effort to schedule any planned medical treatment or supervision so as to minimize disruption to District’s operations. (2
Notice to District. Principals who receive a summons for jury service are to notify Human Resources immediately of the proposed dates of service.
Notice to District. Barring unforeseen circumstances, a unit member must contact the Human Resources Department at least two (2) hours prior to the start of the work day to permit District time to secure a substitute.
Notice to District. If the Leased Premises are damaged or destroyed in whole or in any material part by fire or other casualty, Tenant shall notify District of the same and of the estimated amount of such casualty loss, as soon as reasonably possible after Xxxxxx’s discovery of the damage.

7.6.5.2 The written notice must inform the District of the reasons for the leave, the anticipated duration of the leave and the anticipated start of the leave. The District’s Request for Family and medical Leave Form should be used whenever possible.
7.6.5.3 If a bargaining unit member fails to give 30 days’ notice for foreseeable leave with no reasonable excuse for the delay, the District may deny the family care and medical leave request until at least 30 days after the date the unit member provides notice to the District of the need for family care and medical leave.
7.6.5.4 The bargaining unit member shall consult with the District and make a reasonable effort to schedule any planned medical treatment or supervision so as to minimize disruption to the District’s operations.
Notice to District. CSEA shall notify the District in writing of the identity of its bargaining team member’s representatives, officers, stewards, and CSEA labor relations representative within thirty days after the execution of this Agreement and of their respective authority. CSEA shall also notify the District promptly in writing of any change of designated representatives.
